บทคัดย่อ

ภูมิหลัง: การปิดรูรั่วผนังกั้นหัวใจห้องล่าง (ventricular septal defect, VSD) ชนิด perimembranous (pmVSD)
ผ่านสายสวนหัวใจเป็นทางเลือกที่มีความรุกรานน้อยกว่าการผ่าตัด อย่างไรก็ตาม ข้อมูลจากโรงพยาบาลระดับภูมิภาคในประเทศไทยยังมีจำกัด
วัตถุประสงค์: เพื่อรายงานผลลัพธ์ทางคลินิก ภาวะแทรกซ้อน และความเป็นไปได้ของการปิด pmVSD ผ่านสายสวนหัวใจด้วย Cocoon® VSD Occluder ในผู้ป่วยที่ได้รับการคัดเลือก ณ รพ.ลำปาง
วัสดุและวิธีการ: การศึกษาแบบรายงานผู้ป่วยอนุกรมย้อนหลังในผู้ป่วยต่อเนื่อง 4 รายที่ได้รับการปิด pmVSD ระหว่างเดือนพฤษภาคม พ.ศ. 2565 ถึงกรกฎาคม พ.ศ. 2567 ติดตามผลทางคลินิก echocardiography และ 12-lead ECG ที่ 1, 6 และ 12 เดือน วิเคราะห์ข้อมูลเชิงพรรณนาโดยไม่ทดสอบสถิติเชิงอนุมาน
ผลการศึกษา: ผู้ป่วยทั้ง 4 รายมีอายุเฉลี่ย 31.8 ± 7.0 ปี และประสบความสำเร็จของหัตถการทุกราย TEE ยืนยันการปิดสนิททันทีหลังวางอุปกรณ์ ไม่พบ CAVB, device embolization, new aortic regurgitation หรือ hemolysis ระหว่างติดตาม 12 เดือน ค่า RVSP เฉลี่ยมีแนวโน้มลดลงจาก 43.1 ± 22.4 เป็น 28.9 ± 6.2 มม.ปรอท และ LVIDd เฉลี่ยลดลงจาก 47.8 ± 5.3 เป็น 44.5 ± 3.3 มม.
อย่างไรก็ตาม การเปลี่ยนแปลงรายบุคคลไม่ได้เป็นไปในทิศทางเดียวกันทุกราย ผู้ป่วยทุกรายอยู่ใน NYHA functional class I ตลอดระยะติดตาม และนอนโรงพยาบาลหลังหัตถการ 1 คืน
สรุป: การปิด pmVSD ผ่านสายสวนหัวใจด้วย Cocoon® VSD Occluder ในผู้ป่วยที่ได้รับการคัดเลือกสามารถดำเนินการได้ในโรงพยาบาลภูมิภาคระดับตติยภูมิภายใต้ระบบ
clinical proctorship และไม่พบภาวะแทรกซ้อนสำคัญในผู้ป่วย 4 รายแรก อย่างไรก็ตามควรมีการติดตามระยะยาวและศึกษาในผู้ป่วยจำนวนมากขึ้นเพื่อยืนยันความปลอดภัยและผลลัพธ์ทางคลินิก
