บทคัดย่อ

ภูมิหลัง: การระงับปวดหลังผ่าตัดเปลี่ยนข้อเข่าเทียมอาจยังไม่เพียงพอแม้ใช้การระงับปวดหลายวิธีร่วมกัน การให้ยาแก้ ปวดชนิดรับประทานแบบผู้ป่วยควบคุมเองอาจช่วยให้เข้าถึง ยาได้ทันท่วงที แต่ยังมีข้อมูลจำกัดเมื่อเปรียบเทียบกับการ ให้ยาแก้ปวดทางหลอดเลือดดำโดยพยาบาล 
วัตถุประสงค์: เพื่อประเมินว่าการให้ยาชนิดรับประทานแบบ ผู้ป่วยควบคุม (oral patient-controlled analgesia; oral  PCA) ไม่ด้อยกว่ายาฉีดทางหลอดเลือดดำโดยพยาบาล  (nurse-controlled analgesia; NCA)ในการควบคุมความ ปวดหลังผ่าตัดเปลี่ยนข้อเข่าเทียมข้างเดียว
วัสดุและวิธีการ: การศึกษาแบบ randomized controlled non-inferiority trial ในผู้ป่วยผ่าตัดเปลี่ยนข้อเข่าเทียมข้างเดียวอายุ 50–70 ปี เมื่อต้องการยาแก้ปวด ผู้ป่วยถูกสุ่มให้เป็นกลุ่ม NCA ได้รับมอร์ฟีนทางหลอดเลือดดำ 3 มก. โดยเว้นระยะอย่างน้อย 4 ชั่วโมง หรือกลุ่ม oral PCA รับประทานมอร์ฟีนชนิดออกฤทธิ์ทันทีขนาด 10 มก. โดยเว้นระยะอย่างน้อย 1 ชั่วโมง ผลลัพธ์หลักคือคะแนนความปวดเฉลี่ยขณะพักที่ 36–48 ชั่วโมงหลังผ่าตัด กำหนดขอบเขตความไม่ด้อยกว่าที่ 1.8 คะแนนบนมาตรวัด 0–10
ผลการศึกษา: ผู้ป่วย 64 รายได้รับการสุ่มเข้ากลุ่ม ๆ ละ 32 ราย มี 1 รายในกลุ่ม oral PCA ไม่รับการรักษาตามที่สุ่ม การวิเคราะห์ per-protocol รวม 63 ราย คะแนนความปวดขณะพักเฉลี่ยช่วง 36–48 ชั่วโมงเท่ากับ 3.3±2.0 ในกลุ่ม oral PCA และ 3.2±1.7 ในกลุ่ม NCA ค่าความต่างเฉลี่ย (NCA ลบ oral PCA) 0.0 คะแนน (95% CI −1.0 ถึง 0.9) และช่วงความเชื่อมั่นอยู่ภายในขอบเขตความไม่ด้อยกว่าที่กำหนด กลุ่ม oral PCA ได้รับมอร์ฟีนบ่อยกว่า (มัธยฐาน 3 เทียบกับ 0 ครั้ง; p<0.001) และมีสัดส่วนผู้พึงพอใจระดับมากถึงมากที่สุดสูงกว่า (90.3% เทียบกับ 56.2%; p=0.022) แต่พบคลื่นไส้ อาเจียน และปากแห้งมากกว่า อาการทั้งหมดมีความรุนแรงเล็กน้อยและไม่ต้องออกจากการศึกษา
