บทคัดย่อ
โรคกระดูกสันหลังอักเสบยึดติด (ankylosing spondylitis, AS) เป็นโรคข้ออักเสบเรื้อรังที่ส่งผลต่อกระดูกสันหลัง ข้อสะโพกและข้อต่อกระเบนเหน็บ-เชิงกราน ประมาณร้อยละ 24–36 ของผู้ป่วยมีข้อสะโพกอักเสบและเกิดภาวะข้อสะโพกติดแข็ง จำเป็นต้องได้รับการผ่าตัดเปลี่ยนข้อสะโพกเทียมเพื่อฟื้นฟูการเคลื่อนไหวและลดความพิการ การผ่าตัดในผู้ป่วย AS มีความซับซ้อนสูง เนื่องจากความผิดรูปของกระดูกสันหลังและการเคลื่อนไหวของกระดูกเชิงกรานที่ลดลง การเตรียมตัวผ่าตัดจึงต้องประเมินพิสัยการเคลื่อนไหว ความผิดรูปของกระดูกสันหลัง และภาวะสุขภาพร่วมอื่นอย่างละเอียด การเลือกวิธีผ่าตัดต้องคำนึงถึงลักษณะข้อสะโพกติดแข็งเพื่อแก้ไขโครงสร้างรอบข้อสะโพกได้อย่างเหมาะสม ภาวะแทรกซ้อนที่อาจพบ ได้แก่ กระดูกหักและการเกิด heterotrophic ossification การวางตำแหน่งเบ้าสะโพกเทียมที่เหมาะสม มีความสำคัญต่อการป้องกันข้อหลุด ผลลัพธ์หลังผ่าตัดพบว่าผู้ป่วยมีพิสัยการเคลื่อนไหวและอาการปวดที่ดีขึ้นชัดเจนการดูแลรักษาควรทำแบบสหสาขาวิชาเพื่อเพิ่มความปลอดภัยและประสิทธิภาพของการรักษา
