บทคัดย่อ
ภูมิหลัง: การตรวจลำไส้ใหญ่ด้วยสารทึบรังสี (double-contrast barium enema, BE) จำเป็นต้องเตรียมลำไส้ใหญ่ให้สะอาดก่อนการตรวจ อาจทำได้โดยการกินยาระบายหรือสวนทวาร ยา oral sodium phosphate (OSP) เป็นทางเลือกหนึ่งที่ได้ผลดี สะดวกและนิยมใช้ก่อนการตรวจ colonoscopy แต่ยังไม่เคยนำมาใช้สำหรับการตรวจ BE ในรพ.ศรีสังวรสุโขทัย
วัตถุประสงค์: เพื่อศึกษาความสะอาดของลำไส้ใหญ่ที่เตรียมโดยวิธีกินยา OSP เปรียบเทียบกับการสวนทวารด้วยน้ำอุ่น หรือใช้ทั้ง 2 วิธีร่วมกัน ในการตรวจ BE
วัสดุและวิธีการ: เป็นการศึกษาแบบ prospective randomized clinical trial ในผู้ป่วยที่ได้รับการตรวจ BE ในรพ.ศรีสังวรสุโขทัย ระหว่าง เดือน ก.พ.-ส.ค. 2551 จำนวน 45 ราย แบ่งผู้ป่วยเป็น 3 กลุ่มๆ ละ 15 ราย โดยการสุ่ม กลุ่มที่ 1 ได้รับการสวนลำไส้ใหญ่ด้วยน้ำอุ่น กลุ่มที่ 2 กินยา OSP และกลุ่มที่ 3 สวนลำไส้ใหญ่ ด้วยน้ำอุ่นร่วมกับกินยา OSP บันทึกข้อมูลทั่วไป ปริมาณยาที่ใช้ จำนวนครั้งการสวนและถ่ายอุจจาระ ประเมินความสะอาดของลำไส้ใหญ่และคุณภาพฟิล์มโดยรังสีแพทย์ 1 คน โดยใช้เกณฑ์การประเมินที่ดัดแปลงจาก scoring system for colon cleansing วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา, one way ANOVA และ exact probability test
ผลการศึกษา: ผู้ป่วยที่เข้าร่วมการศึกษาเป็นชาย 18 รายและหญิง 27 ราย อายุเฉลี่ย 52 ปี (พิสัย 24-74 ปี) คุณภาพของภาพรังสีที่นำมาแปลผลและลักษณะทั่วไปของประชากรทั้ง 3 กลุ่มไม่แตกต่างกัน ยกเว้นอายุเฉลี่ยและขนาดรอบเอวในกลุ่มที่ 2 มากกว่ากลุ่มที่เหลือ (p =0.033 และ 0.011 ตามลำดับ) กลุ่มที่กินยา OSP เพียงอย่างเดียวหรือร่วมกับการสวนด้วยน้ำอุ่นมีจำนวนครั้งของการถ่ายมากกว่ากลุ่มที่สวนด้วยน้ำอุ่นเพียงอย่างเดียว (p<0.001) ความสะอาดของลำไส้ใหญ่เมื่อประเมินเป็น 5 ระดับ (grade 0-4) ไม่พบความแตกต่างทั้ง 3 กลุ่ม (p=0.108) แต่หากจำแนกออกเป็น 2 ประเภทคือ ความสะอาดที่อยู่ในเกณฑ์ยอมรับได้ (grade 3-4) และยอมรับไม่ได้ (grade 0-2) พบว่า กลุ่มที่สวนด้วยน้ำอุ่นเพียงอย่างเดียวมีความสะอาดอยู่ใน
เกณฑ์ที่ยอมรับได้น้อยกว่าอีก 2 กลุ่ม (p=0.02) ในขณะที่กลุ่มที่กินยา OSP เพียงอย่างเดียวมีความสะอาดในเกณฑ์ที่ยอมรับได้ไม่แตกต่างจากกลุ่มที่ใช้ทั้ง 2 วิธีร่วมกัน (p=0.651)
สรุป: การกินยา OSP เพียงอย่างเดียวหรือใช้ร่วมกับการสวนทวารด้วยน้ำอุ่นให้ผลความสะอาดของลำไส้ใหญ่ไม่แตกต่างกัน และดีกว่าการสวนด้วยน้ำอุ่นเพียงอย่างเดียว ยาระบาย OSP เป็นทางเลือกที่เหมาะสมในการเตรียมผู้ป่วยสำหรับการตรวจ double-contrast barium enema โดยไม่ต้องสวนทวาร
