ถุงน้ำแคลซิไฟอิง โอดอนโตเจนิค ร่วมกับฟันเขี้ยวฝัง: รายงานผู้ป่วย 1 รายที่ผลการรักษาไม่ธรรมดา

บทคัดย่อ
ถุงน้ำแคลซิไฟอิง โอดอนโตเจนิค เป็นรอยโรคในช่องปากที่พบได้น้อย อาจพบร่วมกับฟันที่ยังไม่ขึ้น รายงานนี้นำเสนอผู้ป่วย 1 รายที่มีถุงน้ำชนิดนี้ร่วมกับฟันเขี้ยวฝังในกระดูกขากรรไกรบน เป็นหญิงไทยโสด อายุ 16 ปี มาด้วยอาการบวมที่แก้มซ้าย 3 สัปดาห์ กดแข็ง ไม่เจ็บปวด เหงือกและกระพุ้งแก้มบวม แต่ไม่มีการอักเสบ ฟันน้ำนมบนซ้ายซี่ 63 ยังไม่หลุด ฟันเขี้ยวแท้บนซ้ายซี่ 23 ยังไม่ขึ้น ถ่ายภาพรังสีพบรอยโรคเป็นเงาดำขอบชัด ขนาด 2 ซม. ภายในมีจุดขาวเล็กๆ ร่วมกับฟันเขี้ยวฝังซี่ 23 รักษาโดยการผ่าตัดถอนฟันน้ำนมซี่ 63 และควักเอาถุงน้ำออกทั้งหมด แล้วปลูกถ่ายฟันเขี้ยวแท้ซี่ 23 มายังตำแหน่งปกติ ต่อมาจึงรักษาคลองรากฟันตามขั้นตอน เมื่อติดตามหลังผ่าตัด 4 ปี พบว่า แผลหายอย่างสมบูรณ์ มีการสร้างกระดูกรองรับรากฟันขึ้นมาใหม่จนเต็ม ฟันเขี้ยวอยู่ในสภาพใช้งานได้ดี ภาพถ่ายรังสีมีการหายของกระดูกขากรรไกรเป็นปกติและไม่พบรอยโรคกลับเป็นซ้ำ
