บทคัดย่อ

การติดเชื้อบริเวณเบ้าตาเป็นภาวะรุนแรงที่อาจนำไปสู่การสูญเสียการมองเห็นและภาวะแทรกซ้อนทางระบบประสาทส่วนกลางได้ แม้ว่าสาเหตุส่วนใหญ่มักสัมพันธ์กับไซนัสอักเสบ
แต่การติดเชื้อจากแหล่งกำเนิดทางทันตกรรมและการติดเชื้อที่สัมพันธ์กับการบาดเจ็บของกระดูกใบหน้า พบได้น้อยและมีความท้าทายในการวินิจฉัย รายงานนี้มีวัตถุประสงค์เพื่อนำเสนอการวินิจฉัยและการรักษาผู้ป่วยติดเชื้อบริเวณเบ้าตา 2 รายที่มีสาเหตุแตกต่างกัน รายแรกเป็นชายสัญชาติเมียนมาอายุ 38 ปี มีการติดเชื้อภายหลังถอนฟันกรามน้อยบนขวาซี่ที่สอง ร่วมกับไซนัสขากรรไกรบนอักเสบ ตรวจเอกซเรย์คอมพิวเตอร์พบโพรงหนองใต้เยื่อหุ้มกระดูกเบ้าตา (subperiosteal abscess) และการอักเสบของเนื้อเยื่อเบ้าตา (orbital cellulitis) ได้รับการผ่าตัดระบายหนองร่วมกับการผ่าตัดเปิดไซนัสขากรรไกรบนทางผนังด้านหน้า (Caldwell–Luc procedure) ส่วนรายที่สองเป็นชายไทยอายุ 25 ปี มีประวัติถูกทำร้ายร่างกายจนเกิดกระดูกเบ้าตาและผนังไซนัสหัก ร่วมกับการอักเสบของเนื้อเยื่อเบ้าตาและโพรงหนองใต้เยื่อหุ้มกระดูกเบ้าตา ได้รับการผ่าตัดระบายหนองร่วมกับการผ่าตัดไซนัสด้วยกล้องส่องผ่านทางจมูก (endoscopic sinus surgery) และการผ่าตัดเปิดไซนัสเอทมอยด์ส่วนหน้า (anterior ethmoidectomy) ผู้ป่วยทั้งสองรายได้รับการผ่าตัดและยาปฏิชีวนะทางหลอดเลือดดำภายใต้การดูแลของสหสาขาวิชาชีพ ผลการรักษาพบว่าอาการติดเชื้อดีขึ้น การมองเห็นและการเคลื่อนไหวลูกตากลับสู่ปกติและไม่พบการติดเชื้อซ้ำในการติดตามผล รายงานนี้เน้นย้ำความสำคัญของการค้นหาแหล่งกำเนิดการติดเชื้อ การตรวจเอกซเรย์คอมพิวเตอร์เพื่อประเมินขอบเขตของโรค และการรักษาแบบสหสาขาวิชาชีพอย่างทันท่วงที ซึ่งมีบทบาทสำคัญต่อการลดภาวะแทรกซ้อนและการคงไว้ซึ่งการมองเห็น
