ความสัมพันธ์ระหว่างค่าดัชนีไตรกลีเซอไรด์-กลูโคสกับภาวะแทรกซ้อนทางหัวใจและหลอดเลือดรุนแรงในผู้ป่วยโรคหลอดเลือดสมองตีบ: การศึกษาแบบย้อนหลังเชิงกลุ่มตัวอย่าง

บทคัดย่อ
ภูมิหลัง: ผู้ป่วยโรคหลอดเลือดสมองตีบมีอัตราการเกิดภาวะแทรกซ้อนทางหัวใจและหลอดเลือดที่รุนแรง (major adverse cardiovascular events; MACE) สูงถึงร้อยละ 15–30 แม้ว่าได้รับการป้องกันทุติยภูมิ การประเมินความเสี่ยงรายบุคคลจึงมีความสำคัญในการวางแผนป้องกัน ดัชนี ไตรกลีเซอไรด์–กลูโคส (triglyceride-glucose index; TyG index) เป็นตัวบ่งชี้ภาวะดื้อต่ออินซูลินที่คำนวณง่าย ใช้ ข้อมูลตรวจพื้นฐานและสัมพันธ์กับความเสี่ยงโรคหัวใจและหลอดเลือด อาจใช้เป็นเครื่องมือช่วยทำนายความเสี่ยงของ MACE ได้ แต่ยังขาดข้อมูลเชิงประจักษ์ในผู้ป่วยโรคหลอดเลือดสมองตีบชาวไทย
วัตถุประสงค์: เพื่อศึกษาความสัมพันธ์ระหว่างค่า TyG index กับความเสี่ยงการเกิด MACE ในผู้ป่วยโรคหลอดเลือดสมองตีบ และระบุค่าตัดที่เหมาะสมสำหรับการจำแนกผู้ป่วยกลุ่มเสี่ยงสูงในบริบทประเทศไทย
วัสดุและวิธีการ: เป็นการศึกษาแบบ retrospective cohort ในผู้ป่วยโรคหลอดเลือดสมองตีบรายใหม่ที่รักษาใน รพ.ป่าซาง จ.ลำพูน ระหว่างเดือนมกราคม พ.ศ. 2562 – ธันวาคม พ.ศ. 2567 จำนวน 281 ราย ตรวจระดับไตรกลีเซอไรด์และกลูโคสขณะอดอาหารเพื่อคำนวณค่า TyG index และติดตามการเกิด MACE ตามนิยาม 3-point MACE ระยะเวลาติดตามมีค่ามัธยฐาน 2.5 ปี ข้อมูลวิเคราะห์ด้วย Cox proportional hazards model และประเมินค่าตัดด้วย ROC curve
ผลการศึกษา: ในผู้ป่วยโรคหลอดเลือดสมองตีบ 281 ราย พบอุบัติการณ์ MACE ร้อยละ 24.2 (68 ราย) กลุ่มที่มีค่า TyG index อยู่ใน tertile สูง (>8.9) มีความเสี่ยงเกิด MACE สูงกว่ากลุ่ม tertile ต่ำ (<8.4) อย่างมีนัยสำคัญ (adjusted HR 2.08; 95%CI 1.23–3.49; p=0.006) การวิเคราะห์ ROC curve พบว่า TyG index มีความสามารถปานกลางในการพยากรณ์ (AUC 0.65; 95%CI 0.57–0.73; p<0.001) โดยค่าตัดที่เหมาะสมที่สุดคือ TyG index ≥ 9.0 ให้ความจำเพาะร้อยละ 80.0 ความไวร้อยละ 45.1 และสัมพันธ์กับความเสี่ยง MACE ที่เพิ่มขึ้น แม้หลังปรับปัจจัยเสี่ยงสำคัญแล้ว (adjusted HR 1.96; 95% CI 1.13–3.41, p=0.017)
สรุป: ค่า TyG index ที่สูงสัมพันธ์กับความเสี่ยงการเกิด MACE ที่เพิ่มขึ้นในผู้ป่วยโรคหลอดเลือดสมองตีบชาวไทย โดยมีจุดตัดที่เหมาะสมที่ ≥9.0 อย่างไรก็ตาม กลุ่มผู้ป่วยที่ศึกษามีค่า TyG index พื้นฐานสูงและช่วงการกระจายแคบ ทำให้ความสามารถในการจำแนกความเสี่ยงอยู่ในระดับปานกลาง จึงควรใช้ TyG index เป็นเครื่องมือเสริมร่วมกับปัจจัยทางคลินิกอื่น มากกว่าการใช้เพียงตัวแปรเดี่ยว
