Results of Simultaneous Bilateral Percutaneous Nephrolithotomy: Series of 45 Thai Patients

Abstract

Background : Percutaneous stone removal has become the treatment of choice for large renal calculi instead of open surgery. Simultaneous bilateral percutaneous nephrolithotomy (SBPCNL) has been demonstrated to be a well–tolerated, safe and cost-effective procedure in large bilateral stone burdens. There was no previous study of SBPCNL in Lampang Hospital.
Objective : To evaluate the safety and efficacy of simultaneous bilateral PCNL in Lampang Hospital.
Material and method : A retrospective study was conducted from January 2009 to December 2010 involving 35 male and 10 female patients with a mean age of 48.0 ± 11.3 years (range, 22 - 81) who underwent SBPCNL in Lampang Hospital by one urologist. Operative time, stone free rate, length of stay, blood transfusion and postoperative complications were evaluated.
Results : Multiple stone and staghorn stone were found in 77.8% and 52.2% of cases. The average size was 31.8 ± 7.8 mm wide (range, 11 - 60) and 45.0 ± 11.9 mm long (range, 10 - 82). The mean operative time was 116 ± 23.6 minutes (range, 50 - 230). The stone free rate was 74.4% and blood transfusion rate was 28.9%. Hospitalization stay was 4.3 ± 2.8 days (range, 1 - 21) taking account of three severe sepsis leading to one death.
Conclusion : Simultaneous bilateral PCNL in properly selected patients and sufficient experienced surgeon was a safe and effective procedure.
