Efficacy of Injection Gun for Medium-Viscosity Bone Cement: Comparison between Invented Gun and Standard Commercial Gun

Abstract

Background: Cemented hip arthroplasty with low viscosity cement has an increased rate of aseptic loosening of femoral components. The injection gun invented from a caulking gun could not withstand the medium or high viscosity cement. We developed a novel gun (Lampang cement gun for medium viscosity cement, LCG-MV) to cope with this problem. 


Objective: To compare the efficacy of an injection gun for medium viscosity bone cement between LCG-MV and standard commercial gun (Optigun®), and also analyze the cost of each gun.


Methods: An experimental study was conducted by applying the Simplex P cement which was injected from both types of guns into a plastic test tube by one surgeon. The injection speed was calculated. The hardened cement was then imaged by CT scan and the bubbles inside were measured. The maximum trigger force and intra-cartridge pressures were measured. The data were compared between groups. The cost of production, both directly and indirectly per one time usage was analyzed.


Results: The LCG-MV and Optigun® had similar injection speed (2.90±0.3 vs 2.95±0.3 g/sec, p=0.362) and intra-cartridge pressure (52.6±2.3 vs 54.4±2.5 psi, p=0.173). Using LCG-MV, the bubble size inside the cement was smaller (1.6±0.5 vs 1.8±0.6 mm, p=0.005) and the number of bubbles with sized ≥1.7 mm were fewer (2±1 vs 3±2 /cm3, p<0.001). The maximum trigger force of LCG-MV was less than the Optigun® (58.8±4.7 vs 305.3±39.7 kg force, p <0.001) and the total cost of production was lower. It accounted for 20–40% of the Optigun®.


Conclusion: The LCG-MV had lower maximum trigger force than the Optigun®, but their intra-cartridge pressure and injection speed were similar. The size of bubbles inside the cement of LCG-MV was smaller.         The lower cost of production may help reduce expenditures when used in clinical applications.
