การศึกษาคุณภาพชีวิตของผู้ป่วยระยะประคับประคองที่ใช้น้ำมันกัญชาขมิ้นทอง ในคลินิกกัญชาทางการแพทย์ จังหวัดอุดรธานี

บทคัดย่อ
บทนำและวัตถุประสงค์: ตำรับน้ำมันกัญชาขมิ้นทอง เป็นภูมิปัญญาการแพทย์แผนไทย พัฒนาโดยแพทย์แผนไทย โรงพยาบาลห้วยเกิ้ง จังหวัดอุดรธานี การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาคุณภาพชีวิตของผู้ป่วยระยะประคับประคองที่ใช้น้ำมันกัญชาขมิ้นทอง
วิธีการศึกษา: เครื่องมือวัดคุณภาพชีวิตประเมินจาก 1) คะแนนความรุนแรงภาวะสุขภาพ (Edmonton Symptom Assessment System; ESAS) 2) แบบสอบถามคุณภาพชีวิตมิติสุขภาพ 5 ด้าน (European Quality of Life 5 Dimensions 5 Level ;EQ-5D-5L) 3) คะแนนสุขภาพทางตรง (Visual Analogue Scale; VAS) 4) ความสามารถในการดูแลตัวเองในชีวิตประจำวันด้านต่าง ๆ ของผู้ป่วยระยะประคับประคอง (Palliative Performance Scale; PPS) รูปแบบการวิจัย แบบกลุ่มเดียวทดสอบก่อนและหลัง บันทึกผลของผู้ป่วยแต่ละรายโดยแพทย์แผนไทยก่อนและหลังการใช้น้ำมันกัญชาขมิ้นทองต่อเนื่องเป็นเวลา 14 วัน ณ วันที่ 0, 7 และ 14 ระหว่างเดือนพฤศจิกายน 2565 ถึงเดือนพฤศจิกายน 2566 จากโรงพยาบาล 6 แห่งและโรงพยาบาลส่งเสริมสุขภาพตำบล 16 แห่งในจังหวัดอุดรธานี วิเคราะห์ด้วยสถิติพรรณนาและ Friedman test
ผลการศึกษา: ผู้ป่วยประคับประคอง 122 ราย เพศชายร้อยละ 61 อายุเฉลี่ย 68±12 ปี เป็นโรคมะเร็งร้อยละ 32, โรคหลอดเลือดสมองร้อยละ 14, ความสามารถในการดูแลตัวเองส่วนใหญ่ร้อยละ 68 อยู่ในระดับคงที่ (PPS>70) ข้อบ่งใช้ของน้ำมันกัญชาขมิ้นทองคือนอนไม่หลับ (ร้อยละ 55) รองลงมาคืออาการปวด (ร้อยละ 21) และเบื่ออาหาร (ร้อยละ 16) ตามลำดับ คะแนนความรุนแรงภาวะสุขภาพ (ESAS) พบว่าอาการปวด, เหนื่อยเพลีย, คลื่นไส้, เบื่ออาหาร, ไม่สบายกายใจ และอาการนอนไม่หลับ ลดลงอย่างมีนัยสำคัญทางสถิติ (p < 0.05) มิติสุขภาพ 5 ด้าน (EQ-5D-5L) พบว่ามิติด้านการเคลื่อนไหวและวิตกกังวลซึมเศร้าดีขึ้น (p < 0.05) คะแนนประเมินสุขภาพทางตรง (VAS) เพิ่มขึ้น (p = 0.01) ทั้งนี้คะแนนอรรถประโยชน์มิติสุขภาพและระดับความสามารถในการดูแลตัวเอง (PPS) ก่อนใช้กับหลังใช้ยาไม่แตกต่างกัน (p > 0.05) การใช้ยาไม่มีผลเปลี่ยนแปลงสัญญาณชีพ เช่นความดันโลหิตหรือชีพจร มีรายงานอาการไม่พึงประสงค์ 1 รายคือปากคอแห้ง
อภิปรายผล: ผลการศึกษานี้สอดคล้องกับการศึกษาก่อนหน้าที่ยากัญชาช่วยบรรเทาอาการของผู้ป่วยกลุ่มนี้ ยกระดับคุณภาพชีวิตและมีความปลอดภัย แต่อย่างไรก็ตามผู้ป่วยกลุ่มที่ศึกษายังถือว่ามีอาการค่อนข้างคงที่ (PPS score >70) ซึ่งอาจจะแตกต่างจากการกลุ่มผู้ป่วยในการศึกษาก่อนหน้า
ข้อสรุปและข้อเสนอแนะ: น้ำมันกัญชาขมิ้นทอง เพิ่มคุณภาพการนอนหลับ บรรเทาอาการปวด และทำให้ผู้ป่วยรับประทานอาหารได้เพิ่มขึ้น ควรมีการศึกษาเชิงลึกทางคลินิกในผู้ป่วยระยะประคับประคองในระยะเวลาที่นานขึ้น
