Factors Related to Coping Difficulties after Hospitalization in Patients with Closed Leg Fracture Undergoing Open Reduction Internal Fixation Surgery

Abstract
Coping difficulties after hospitalization discharge have an impact on a full recovery of each individual patient with a closed leg fracture and having open reduction internal fixation (ORIF) surgery. This descriptive study aimed to explore the relationship between discharge readiness, pain, and anxiety related to coping difficulties after hospitalization. Eighty four patients with closed leg fracture undergoing ORIF at Chonburi hospital were recruited using specified time period sampling. Data collection was conducted during March to December, 2018. Research instruments included Readiness for Hospital Discharge Scale, The Post Discharge Coping Difficulties Scale, Brief Pain Inventory, State-Trait Anxiety Inventory form Y-1 with the reliabilities of .82, .80, .87 and .85 respectively. Descriptive statistics and Pearson’s product moment correlation coefficients were employed for data analyses.
The results of this study showed that the sample had a high level of discharge readiness (M = 162.98, S.D. = 25.86), and low levels of pain (M =1.9, S.D. =1.29), anxiety (M =28.69, S.D. = 5.57) and coping difficulties after hospitalization (M = 23.7, S.D. =12.19). Discharge readiness had negative and moderate association with coping difficulties after hospitalization at significant p value of .01 (r = -.34). Pain and anxiety were related to coping difficulties after hospitalization at significant p value < .01 (r = .58 and r = .57, respectively)
The study results suggested that nurses and health care providers should assess and promote patients’ discharge readiness. Management of pain and anxiety in these patients during and post hospitalization as well as in a recovery period after discharge are also recommended. These could enhance successful recovery throughout their transition period after hospital discharge and quality of life among these patients.
