The Effective of Using Clinical Nursing Practice Guideline for Patients with Intraocular Lens Implants to Prevent Endophthalmitis Infection in Mae Sot Hospital

Abstract

This quasi-experimental study aimed to (1) examine the situation and context of implementing Clinical Nursing Practice Guidelines (CNPG) to prevent endophthalmitis in patients receiving intraocular lens implants at Mae Sot Hospital, (2) develop CNPG to prevent endophthalmitis in such patients, and (3) evaluate the effectiveness of the newly developed CNPG. Research Methodology: This quasi-experimental study utilized the framework of the ADAPTE Collaboration consists of (1) set up phases 2) adaptation phase and 3) finalization phase). The sample consisted of 11 nurses working in the cataract surgery operating room who had knowledge and understanding of sterile techniques, infection prevention, and cataract surgery instrument handling, with at least one year of training and experience assisting in cataract surgeries. Additionally, 98 patients diagnosed with cataracts and scheduled for cataract surgery with intraocular lens implantation at Mae Sot Hospital between October and December 2023 were included. Research instruments included (1) CNPG to prevent  endophthalmitis in patients receiving intraocular lens implants, (2) a checklist to monitor compliance with the CNPG, consisting of 21 items, (3) a satisfaction questionnaire, and (4) a tracking form for infection incidents and complications within 24 hours post-surgery. Data were analyzed using descriptive statistics and percentage differences.


Results: The CNPG to prevent endophthalmitis in patients receiving intraocular lens implants, suitable for the hospital's context. After implementing the guidelines, 95.23% of the operating room nurses followed the new CNPG accurately and comprehensively, an improvement from the previous 76.19%. Nurses' satisfaction with the updated CNPG was at a Strongly agree (  = 4.58, SD = .34). Additionally, no cases of endophthalmitis or complications were reported within 24 hours post-surgery among the cataract patients.


Conclusion: The newly developed CNPG is effective in preventing endophthalmitis in patients receiving intraocular lens implants.


Implications: The newly developed CNPG can be implemented to enhance patient safety and surgical outcomes.
