Development of a Continuous Bladder Irrigation Model in Patients Undergoing Tranurethral Resection of the Prostate Gland, Thabo Crown Prince Hospital, Nongkhai Province

Abstract
This research and development study aimed to investigate the conditions, develop a model, and evaluate the effectiveness of continuous bladder irrigation in patients undergoing transurethral resection of the prostate (TURP). The sample consisted of 27 patients with benign prostatic hyperplasia who underwent TURP at Thabo Crown Prince Hospital, Nong Khai Province, selected by purposive sampling. The research instruments included: 1) a post-operative pain assessment form, 2) a post-operative complication assessment form after returning from the operating room, and 3) a satisfaction assessment form. Content validity was examined by three experts, yielding IOC values ranging from .67 to 1.00 and a reliability coefficient (Cronbach’s alpha) of .77 Data were analyzed using descriptive statistics and One sample t-test.
The research findings were as follows; 1. From 2021 to 2023, there has been a continuous increase in the number of patients undergoing Transurethral Resection of the Prostate (TURP) procedures with Continuous Bladder Irrigation (CBI). This rising trend presents several challenges for professional nurses in preventing post-operative complications. registered nurses expressed a need for practical, applicable clinical guidelines for caring for patients with benign prostatic hyperplasia (BPH) receiving continuous bladder irrigation (CBI), with provisions for ongoing monitoring and systematic outcome evaluation. 2.The development of a continuous bladder irrigation care model for patients undergoing transurethral resection of the prostate (4P model) consisted of: 1) Pre-operative Evaluation, 2) Performing Continuous Bladder Irrigation, 3) Post-operative Evaluation after discontinuing CBI, and 4) Preparation Discharge Planning. 3. After implementing a new structured nursing care model: Pain scores decreased significantly (p<.001), No major complications occurred, with only 25.9% of patients experiencing post-operative fever within 24 hours , Patient satisfaction expressed over 90% at the p<.001 level.
Therefore, the 4P Model should be established as the standard practice guideline for Continuous Bladder Irrigation (CBI) in patients undergoing Transurethral Resection of the Prostate (TURP). The protocol provides a unified approach for nurses to follow, ensuring consistency in patient care and optimal outcomes.
