การเปรียบเทียบการจัดสรรค่าใช้จ่ายบริการสาธารณสุขกองทุนหลักประกันสุขภาพแห่งชาติ ของโรงพยาบาลส่งเสริมสุขภาพตำบลก่อนและหลังถ่ายโอนไปองค์การบริหารส่วนจังหวัดภูเก็ต

การวิจัยผสมวิธีนี้เพื่อ 1) ศึกษาสถานการณ์และเปรียบเทียบสัดส่วนการจัดสรรค่าใช้จ่ายบริการสาธารณสุขกองทุนหลักประกันสุขภาพแห่งชาติ และ 2) เปรียบเทียบต้นทุนต่อหน่วยการจัดบริการสาธารณสุขของ รพ.สต. ก่อนและหลังถ่ายโอนไปองค์การบริหารส่วนจังหวัด (อบจ.) ดำเนินการ 2 ขั้นตอน ขั้นตอนที่ 1 ศึกษาสถานการณ์และเปรียบเทียบสัดส่วนการจัดสรรค่าใช้จ่ายเป็นการศึกษาเชิงคุณภาพและเชิงปริมาณ โดยคัดเลือก รพ.สต. แบบเฉพาะเจาะจงตามขนาดเล็ก กลาง และใหญ่ จำนวน 3 แห่ง และคัดเลือกผู้รับผิดชอบเงินค่าใช้จ่ายเพื่อบริการสาธารณสุข จำนวน 10 คน เก็บข้อมูลเชิงคุณภาพโดยใช้แบบสัมภาษณ์เชิงลึกและวิเคราะห์เชิงเนื้อหา ข้อมูลเชิงปริมาณใช้แบบบันทึกจัดสรรค่าใช้จ่ายเพื่อบริการสาธารณสุข วิเคราะห์ข้อมูลโดยใช้จำนวนและร้อยละ ขั้นตอน ที่ 2 การเปรียบเทียบต้นทุนต่อหน่วยการจัดบริการสาธารณสุข ปีงบประมาณ 2565 และ 2566 ประกอบด้วยข้อมูลต้นทุนค่าแรง ค่าวัสดุ และค่าครุภัณฑ์ โดยใช้แบบเก็บข้อมูลต้นทุนจัดบริการสาธารณสุขในระดับปฐมภูมิ วิเคราะห์ข้อมูลโดยจำนวน ร้อยละ และค่าเฉลี่ย ผลวิจัยพบว่า
1. สถานการณ์การจัดสรรค่าใช้จ่ายเพื่อบริการสาธารณสุขก่อนถ่ายโอน ขึ้นอยู่กับข้อตกลงของคู่สัญญาบริการสาธารณสุขระดับปฐมภูมิที่มีการจัดสรรเป็นตัวเงิน เวชภัณฑ์ยาและเวชภัณฑ์มิใช่ยา หลังถ่ายโอนการจัดสรรเป็นไปตามข้อตกลงของคณะกรรมการสุขภาพระดับพื้นที่ (กสพ.) มีการปรับเปลี่ยนรูปแบบของการจัดสรรเงิน เช่น โอนตรงจากสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.) ในกลุ่มการจัดบริการสร้างเสริมสุขภาพและป้องกันโรค (P&P) หลังการถ่ายโอน มูลค่าการจัดสรรยาและเวชภัณฑ์ที่มิใช่ยาลดลง 13,140.63 - 160,643.74 บาท (ร้อยละ 8.97 - 17.95) แต่มูลค่าเงินได้รับจัดสรรจาก สปสช. เพิ่มขึ้น 26,506.25 – 1,540,597.27 บาท (ร้อยละ 57.10 – 329.10)
2. ต้นทุนรวมการจัดบริการสาธารณสุขก่อนถ่ายโอน 13,279,044.06 บาท หลังถ่ายโอน 12,992,762.78 บาท หลังถ่ายโอนต้นทุนค่าแรงเพิ่มขึ้น 893,622 บาท (ร้อยละ 10.25) ต้นทุนต่อหน่วยของกิจกรรมส่วนใหญ่ มีแนวโน้มเพิ่มขึ้น เช่น การส่งเสริมสุขภาพหญิงตั้งครรภ์ ก่อนการถ่ายโอน มีค่าเฉลี่ยต้นทุน เท่ากับ 1,183.16 บาท และหลังการถ่ายโอนมีค่าเฉลี่ยต้นทุนเท่ากัน 1,810.97 บาท
รูปแบบการจัดสรรอยู่ในระยะเปลี่ยนผ่านของการถ่ายโอนที่ควรมีการเฝ้าระวังต้นทุนการจัดบริการสาธารณสุข โดยควบคุมและติดตามต้นทุนให้อยู่ในระดับมาตรฐานตามขนาดของโรงพยาบาลส่งเสริมสุขภาพตำบล เพื่อไม่ให้เกิดผลกระทบทางงบประมาณและการจัดบริการในสถานบริการต่อไป
