Children with Congenital Heart disease in Primary Care Unit: A Continuing Care at Home (CCaH) Program and its Effects on Motivation, Hygiene Factors, as well as Satisfaction of Nurses

Abstract

The purposes of this research and development study were to develop a continuing care at home (CCaH) program for children with congenital heart disease (CHD), and to examine its effectiveness on motivation and hygiene factors, as well as the satisfaction of nurses. The research was divided into 3 phases. Phase 1 was a study of the problems encountered by nurses in primary care unit while caring for children with CHD, using focus groups. Phase 2 was the development of the program by analyzing data and reviewing relevant literature, ending with the program trial. Phase 3 was the comparison of motivation and hygiene factors and satisfaction of nurses using quasi-experimental research (one group pre-posttest design). The sample consisted of 30 subjects, selected by multistage random sampling technique. Research instrument was the program of continuing care at home for children with CHD. Data were collected using a questionnaire of motivation and hygiene factors, and satisfaction with the program of continuing care at home for children with CHD. Data were analyzed using descriptive statistics and paired-sample t-test.

The research results showed the following.

1. The continuing care at home (CCaH) program for children with CHD in primary care unit consisted of teaching, by using a CHD pediatrics care manual for nurses. The program includes activities to assess, analyze and forward the health status of children with CHD, using both the health monitoring record of children with CHD and the Line application. Demonstration activities to caring for children with CHD during home visits, using CHD pediatrics care manual of caregivers, and activities for exchanging experiences in caring for children with CHD, are also included, both for group discussion and the Line application.

2. Nurses had significantly higher mean scores of motivation and hygiene factors than before receiving the program (t=3.98, p<.001), and the mean score of satisfaction after receiving the program was significantly higher than before receiving the program (t=4.29, p<.001).

Nurses in primary care unit should receive preparation into in caring for children with CHD. Such a preparation should include teaching, demonstrations during home visits, and a focus group for exchanging experiences so they are able to improve their knowledge and skills. This program encourages satisfaction with the job and have incentives to continually develop care for children with CHD in the community.
