Nurse’s Roles in a Primary Medical Care Unit (Health Region 11)

Abstract

The purposes of this mixed methods study were to: 1) describe nurse’s roles regarding primary medical care, and 2) study both supportive and barrier factors of working in a primary care unit. This study was a part of the Thailand Nursing and Midwifery Council Research Project, entitled “Roles and Responsibilities of Nurses in Driving Primary Health Care Systems”. Several primary care units in Health Region 11 were randomly selected. Participants were 46 nurses, recruited using purposive sampling. Instruments consisted of a questionnaire with different parts, such as nursing role, observation form, and semi-structured interview questionnaire about primary medical care and factors related to nursing roles. Data were analyzed by descriptive statistic, Mann-Whitney U-test, and content analysis. Results showed as follows.

1. Most of nurses (95.70 %) were nurse practitioners with mean age 43.85 years (SD=7.90). Their roles regarding primary medical care was at the highest level; the highest mean score was in health assessment, followed by giving health education to patients and families.

2. Overall, the primary medical care roles were not significantly different in primary care unit and primary care cluster. However, the role in medical diagnosis was significantly different (p<.05).

3. The herbal medicines were usually used for primary medical care were andrographis panicolata, senna, and turmerin.

4. The barriers factors related to primary medical care were insufficiency of medical use for treatment, inappropriate number of nurses and having multiple workloads as well as patients’ preference in antibiotic use.

Therefore, nurse competency in primary medical care should be developed continuously. An analysis of nurse workload and the appropriate allocation of health personnel as well as public awareness of using antibiotics and herbal medicines should be concerned.
