Abstract

Septic shock due to severe sepsis is a critical condition. Patients with septic shock are in a life-threatening emergency and require immediate medical attention. Sepsis is when the infection reaches the bloodstream and causes inflammation in the body. As a result, sepsis could ultimately progress to multiple organ dysfunction syndrome and remains a leading cause of death. Therefore, prompt diagnosis and early nursing intervention in the initial stage is very important to prevent complications and to reduce mortality rates from septic shock. This process includes taking the patient history as well as nursing assessment. Moreover, choosing an appropriate assessment tools is a vital decision in the process of early detection of septic shock. The assessment of Search out Severity (SOS), quick Sequential Organ Failure Assessment (qSOFA), Sequential Organ Failure Assessment (SOFA), and Modified early warning signs [MEWs] are recommended as a part of screening for suspicion of sepsis. In short, early detection of sepsis will help to decrease the mortality rates from septic shock. This article describes how assessing patient’s sepsis and septic shock and how to provide proper care. Nurses must perform early detection of sepsis. Delivering high quality nursing care and use the appropriate assessment tools can be effective in helping patients to survive from very serious life-threatening illness.
