Outcomes of Promoting Rational Drug Use of Metformin in Patients with Type 2 Diabetes in Khokphochai Hospital, Khon Kaen

Abstract
Introduction: Promoting the use of metformin in patients with type 2 diabetes is the part of all indicators in rational drug use hospital program. The objective of this study was to determine the indicator and therapeutic outcomes of promoting rational use of metformin in patients with type 2 diabetes.
Methods: Mixed methods research was conducted which a quantitative research was conducted retrospectively collecting data in type 2 diabetic patients with uncontrolled fasting blood sugar in Khokphochai Hospital between 2018, October to 2020, September. The participants were assigned into two groups who received or were not encouraged to use metformin. The proportion of patients with type 2 diabetes who used metformin alone or in combination with other drugs for controlling blood sugar level was followed throughout the year. Each group of 144 patients was monitored fasting blood sugar level for 6 months. The qualitative study was conducted interviews with 9 physician and nurses to determine the factors contributing to the rational prescribing of metformin in type 2 diabetic patients.
Results: The results showed that the proportion of patients with type 2 diabetes who used metformin was as high as 92.34%. The metformin promoted group had statistic significant lower blood sugar level (147.72 mg/dl) than the non-metformin promoted group (161.21 mg/dl). Factors associated with rational prescribing of metformin include; there is a policy to operate the hospital project that promotes rational drug use at the national level, provincial level, and hospital level. There are personnel and committees that are primarily responsible. Tools and technologies are used together, and there is a system for monitoring and processing by provincial networks.
Conclusion: In summary, patients with type 2 diabetes who have no contraindications to metformin should be encouraged to use it as it can improve their glycemic control.
