THE EFFECTIVENESS OF HOME HEALTH CARE PROGRAM ON LENGTH OF HOME STAY IN SCHIZOPHRENIA PATIENTS IN BANGKOK

Objective: To study the effectiveness of home health care program on length of home stay in schizophrenia patients in Bangkok.
Material and method: This was quasi-experimental study on two group with pre and post-test design. The subjects which selected by purposive sampling and then divided into control and experimental group were 56 schizophrenic patients who readmitted within 28 days at Somdet Chaopraya Institute of Psychiatry during October 2013 - September 2014, and they live in Bangkok. The control group received treatment as usual. The experimental group received Home Health Care program. The instruments were the Brief Psychiatric Rating Scale (BPRS), Quality of life measurement of psychiatric patients, the burden of care questionnaire, and length of home stay. The data were analyzed using descriptive statistics for mean, percentage and comparative data on length of home stay of control and experimental group using inferential statistics for unpaired t-test.
Results: After home health care program, the mean score of length of home stay among control group was 126.41 days while the experimental group was 233.78 days. It revealed that length of home stay among experimental group was significantly longer than those of control group 107.37 days (p = 0.005). The patients who received program had decrease psychotic symptoms (74.07%) and high quality of life level (x = 2.87, SD = 0.93). Their care givers had moderate burden of care (x = 46.28, SD = 16.12).
Conclusion: Findings suggested that this Home Health Care program in Bangkok should be recommended to train family and patients for schizophrenic patients caring skill development including community and health center participation. This could help schizophrenia patients and caregivers ability increasing both sustainable living and good quality of life without burden feeling.
