ปัจจัยที่มีผลต่อการเลี้ยงลูกด้วยนมแม่อย่างเดียว 6 เดือนแรกหลังคลอดของมารดาวัยรุ่น ในจังหวัดนครสวรรค์

การวิจัยแบบบรรยายเชิงทำนายนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่มีผลต่อการเลี้ยงลูกด้วยนมแม่อย่างเดียว 6 เดือนแรกหลังคลอดของมารดาวัยรุ่นในจังหวัดนครสวรรค์ กลุ่มตัวอย่างเป็นมารดาวัยรุ่นในจังหวัดนครสวรรค์ จำนวน 166 คน สุ่มตัวอย่างแบบง่ายจากทะเบียนมารดาหลังคลอดวัยรุ่น ตั้งแต่เดือนพฤษภาคมถึงสิงหาคม พ.ศ. 2566 เครื่องมือในการรวบรวมข้อมูล ใช้แบบสอบถาม 6 ฉบับ ประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความรู้เกี่ยวกับการเลี้ยงลูกด้วยนมแม่ แบบสอบถามความตั้งใจในการเลี้ยงลูกด้วยนมแม่ แบบสอบถามทัศนคติต่อการเลี้ยงลูกด้วยนมแม่ แบบสอบถามการคล้อยตามกลุ่มอ้างอิง และแบบสอบถามการรับรู้การควบคุมพฤติกรรม โดยแบบสอบถามความรู้เกี่ยวกับการเลี้ยงลูกด้วยนมแม่ผ่านการทดสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ และทดสอบความเที่ยงโดยใช้วิธีคูเดอร์-รีชาร์ดสัน 20 (KR-20) ได้ค่าเท่ากับ .65 ส่วนแบบสอบถามอีก 4 ชุด ทดสอบความเที่ยงโดยวิธีหาค่าสัมประสิทธิ์แอลฟาของครอนบาค(Cronbach’s Alpha Coefficient) มีค่าความเชื่อมั่นเท่ากับ .78, .77, .82 และ .80 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติถดถอย โลจิสติก
ผลการศึกษาพบว่า อายุ ระดับการศึกษา ลักษณะครอบครัว รายได้ครอบครัว ความรู้เกี่ยวกับการเลี้ยงลูกด้วยนมแม่ ความตั้งใจในการเลี้ยงลูกด้วยนมแม่ ทัศนคติต่อการเลี้ยงลูกด้วยนมแม่ การคล้อยตามกลุ่มอ้างอิง และการรับรู้การควบคุมพฤติกรรม ร่วมกันทำนายการเลี้ยงลูกด้วยนมแม่อย่างเดียว 6 เดือนแรกหลังคลอดของมารดาวัยรุ่นได้ 41% (NagelkerkeR2 = 0.41) ปัจจัยที่มีผลต่อการเลี้ยงลูกด้วยนมแม่อย่างเดียวของมารดาหลังคลอดวัยรุ่นอย่างมีนัยสำคัญทางสถิติ ได้แก่ อายุ รายได้ครอบครัว ความตั้งใจในการเลี้ยงลูกด้วยนมแม่ ทัศนคติต่อการเลี้ยงลูกด้วยนมแม่ การคล้อยตามกลุ่มอ้างอิง และการรับรู้การควบคุมพฤติกรรม (OR = 4.24, 95%CI = 1.61, 11.19, p <.01; OR = 2.59, 95%CI = 1.02, 6.57 p <.05; OR = 2.43, 95%CI = 1.06, 5.57, p <.05; OR = 3.36, 95%CI = 1.42, 7.97, p <.01; OR = 2.79, 95%CI = 1.19, 6.55, p <.05; OR = 3.47, 95%CI = 1.41, 8.58, p <.01 ตามลำดับ) ดังนั้นผู้ประกอบวิชาชีพด้านการดูแลสุขภาพ โดยเฉพาะพยาบาลผดุงครรภ์ ควรประเมิน อายุ รายได้ครอบครัวความตั้งใจในการเลี้ยงลูกด้วยนมแม่ ทัศนคติต่อการเลี้ยงลูกด้วยนมแม่ การคล้อยตามกลุ่มอ้างอิง และการรับรู้การควบคุมพฤติกรรมในการเลี้ยงลูกด้วยนมแม่ตั้งแต่ในระยะตั้งครรภ์ เพื่อให้มารดาหลังคลอดวัยรุ่นมีความมั่นใจที่จะเลี้ยงลูกด้วยนมแม่ต่อเนื่องได้
