Stroke Care Management Model to Practice in Theon District, Lampang Province

Abstract

        Background: There are up to 310 stroke patients in Thoen district so it was necessary to develop the system to take care the stroke patients. This resulted to the best quality of the patient themselves, their family and community.

          Purpose: To study the uses of lesson learning from the stroke patients to practice.

          Method: The qualitative study was performed. The sample group was the 19 registered nurses in charge of take care of stroke patient from sub-district health promotion hospital and Thoen Hospital in Thoen, Lampang Province. The sample group presented each 5 cases-studied and totally 95 cases to analyse the models and criticized the detail components.

          Result: There are 7 stroked care management models were investigated such as
1) Assessment which were the patient’s need, the family’s need, the existed resources and service system 2) Patient management which were the secondary prevention, habitat improvement, recovery, stress elimination and relationship among the people. 3)Family management which were training the caregivers, stress elimination, intermittent, access to financial support and individual relationship. 4) Community management which were transfer, training of volunteer,  financial and tools support 5) Multi-occupation team which were training of Multi-occupation team, tool and methodology support, developed service and service.
6) Transfer Development 7) Assessment which were both patient and family evaluation, system and impact evaluation.

          Conclusion and Suggestion: To take care of stroke patients must be the specific model and clear, practicable and simply for the best quality of patients and relatives.
