Participatory Action Research Results for Developing an Emergency Notification System for Stroke Patients in Mueang Samut Prakan District, Samut Prakan Province

Abstract

         This participatory action research aimed to (1) develop suitable emergency notification guidelines for the public regarding stroke patients and (2) evaluate the stroke knowledge of residents in Mueang Samut Prakan District, Samut Prakan Province, as well as notify emergency medical services (EMS) when they see someone having a stroke. We purposively selected two groups of participants: 20 stakeholders and 440 citizens. The research followed a four-step participatory action research process: planning, acting, observing, and reflecting. Research instruments included in-depth interviews, personal information questionnaires, stroke knowledge questionnaires, and emergency notification knowledge questionnaires via the EMS system. Quantitative data were analyzed using descriptive statistics and the Wilcoxon signed-rank test, while qualitative data were analyzed using content analysis.
       The research findings revealed that the existing guidelines were not aligned with the local context, exhibiting limitations in public knowledge about stroke, understanding of emergency notification, and access to emergency medical services (EMS). Through the participation of stakeholders, new guidelines were designed, incorporating the BEFAST principle for initial symptom assessment and adjusting the content to suit the community context. Additionally, a "Community Research Mentoring Team" was assigned to continuously support the implementation of the guidelines at the local level. Regarding knowledge assessment, the sample group showed a statistically significant increase (p<0.05) in their knowledge about stroke and emergency notification. Their scores for knowledge about emergency notification via the EMS system were also high. Reflection with stakeholders indicated that the public had a better understanding and could report emergencies more accurately. Suggestions included developing promotional media in the form of infographics and video clips to disseminate knowledge through online channels, extending these guidelines to other chronic disease groups, and increasing cooperation with local administrative organizations and schools as local emergency reporting hubs.
