Factors in the Roles of Nurse Affecting that Sustainable Service for Chronic Disease Patients in the Primary Health Care Center

Abstract

          The descriptive research aimed to examine nursing roles within the chronic care service to promote sustainability as well as service aspects affecting nursing duties for patients with chronic illnesses. In a primary healthcare center, 289 participants, including healthcare professionals, health volunteers, and patients with chronic illness, were recruited using a purposive sampling approach. The seven service providers consisted of doctor, pharmacist, registered nurse, medical technologist, public health technical officer, physiotherapist and nutritionist and 43 public health volunteers in the primary health care center, including 239 chronic disease patients at primary health care center. Using content validity and Cronbach’s alpha coefficients of 0.94 and 0.93, respectively, three experts evaluated the survey’s sustainable services and nursing role questions for chronic care services.

            The result showed that the overall nursing role and the sustainable service of chronic care was at high level. Six aspects including healthcare organization, delivery system, decision support, self-management support, clinical information system, and community of nursing responsibilities showed a positive correlation with chronic care patients’ service sustainability. The sustainable services including delivery system, clinical information, health care organization, and self-management support was statistically significant (p <.05). The service of chronic care at a primary healthcare center should be supported and managed by a nurse in accordance with their responsibility to conduct their activity continuously and sustainably.
