Implementing Interprofessional Education via Home Visits Involving Nursing and Pharmacy Students

Abstract

         This research and development study aims to design and implement an interdisciplinary curriculum for nursing and pharmacy via home visit in community and this Interprofessional Education (IPE) program focused on health care for older patients with chronic diseases as well as collaborative practice as a member of an interdisciplinary team. The interprofessional students visited older patients with chronic diseases in community twice to provide health care and health education related with nursing science and pharmacy science based on Project - based Health Care Development Practicum and Public Health Pharmacy subject. Finally, evaluation by group and individual reflection was conducted to measure the effectiveness of the IPE program.
        The result showed that Interprofessional Education Via Home Visits model should consider course design, student and teacher capacities and implementing IPE intra-curriculum via home visits provided an opportunity for nursing and pharmacy students as well as their instructors to build relationships and expand their understanding of each other’s role and responsibility. Significantly, the knowledge of one’s own role and those of other professions was applied to appropriately assess and address the health care needs of older patients with chronic diseases.
