The Influence of Indirect Costs on Access to Rehabilitation Services Among Intermediate Care Patients in Mueang Chaiyaphum District

This study employed a cross-sectional analytical research design to examine the influence of indirect costs on access to rehabilitation services among intermediate care patients in Mueang Chaiyaphum District. The study included all 172 intermediate care patients; therefore, no random sampling was conducted. Data was collected between December 1, 2025, and January 9, 2026, using a structured interview questionnaire. Content validity was assessed by three experts, with all items yielding a content validity index greater than 0.50 and a reliability coefficient of 0.97. Descriptive and inferential statistics were used for data analysis, including multiple logistic regression, with a statistical significance level set at 0.05.
The results showed that 122 intermediate care patients completed the questionnaire. Of these, 69 were female (56.56%), with a mean age of 63.78 years (S.D. = 16.09). Sixty-one participants (50.00%) were unemployed or had no occupation, and 101 (82.79%) resided outside the municipal area. The average food expenditure was 205.66 baht (S.D. = 135.37). Fifty-six participants (45.90%) had resigned from or lost their jobs due to their condition. Only 37.70% of patients completed rehabilitation services within six months. Social determinants of health and indirect cost factors significantly associated with access to rehabilitation services included food and beverage expenditure (ORadj = 10.11, 95% CI: 1.41–27.30), loss of income (ORadj = 3.63, 95% CI: 1.34–9.85), and occupation as a company employee, general laborer, or self-employed trader (ORadj = 3.20, 95% CI: 1.15–8.93).
The findings indicate that indirect costs play a significant role in access to rehabilitation services. These results should be applied to inform policy planning aimed at reducing financial burdens and improving equitable access to rehabilitation services for intermediate care patients.
