การพัฒนารูปแบบการสื่อสารทางการพยาบาลแบบไร้รอยต่อโดยใช้เทคนิค SBAR ในระยะเปลี่ยนผ่านการดูแล งานผู้ป่วยอุบัติเหตุและฉุกเฉิน โรงพยาบาลมหาราชนครราชสีมา Development of a Seamless Nursing Communication Model Using SBAR Techniques during Transitional Care Period, Emergency Nursing Division, Maharat Nakhon Ratchasima Hospital

Abstract

This action research aimed to 1) develop a seamless nursing communication (SNC) model guided by SBAR technique during transitional care period at Emergency Nursing Division, Maharat Nakhon Ratchasima Hospital and 2)study the effect of SNC model on the quality of the communication, handoff communication time, and satisfaction.  This research was implemented between March 2017 to April of 2018. The participant composed of 51 emergency nurses and 436 emergency patients. Research instruments included 1) SNC model, 2) SBAR record sheet, 3) the 4 Cs communication checklist, 4) time record sheet, 5) Nurses’ satisfaction with SNC model questionnaire, and 6) Focus group discussion. Data were analyzed using descriptive statistics, Chi-square test, independent t-test, and content analysis. The findings showed that after SNC model was employed the quality of communication in 4 parameters were improved. There were statistically significant different about the communication quality, Correct and Concise, before and after the intervention (χ²=22.77 and 41.64, p <.001), respectively. There was no statistical significant difference in the communication quality for Complete, and Clear, before and after the intervention (χ²= 1.75 and 1.15, p>.05), respectively. Time consuming for communication was statistically significant reduced after employing the SNC model (before the 6.1 minutes/case, after 3.5 minutes/case) (t = 29.87, p < .001). In addition, the nurses’ satisfaction with SNC model was in high level (Mean = 32.63, SD = 3.75).

In conclusion, this SNC model was the innovation focusing on system outcome, professional outcome, and clinical outcome. This seamless nursing communication model guided by SBAR is efficient. SNC model provides an organized, logical sequence and improved communication. Therefore, it can be recommended for all healthcare settings.
