The care accessibilities of people with alcohol-use disorder (AUD) and the health service system readiness

Objective: To study the care accessibility of people with alcohol-use disorder (AUD) and the readiness of health service system in general and community hospitals.
Methods: The data from the national health data center (HDC) were tracked from 2017-2019. The readiness for alcohol service questionnairs according to the alcohol service guideline were distributed to all level of hospitals under the Ministry of Public Health. The data from the hospitals in 12 provinces representing the 12 health regions were seleced to analyzed.
Results: The accessiblity rate of health care among people with AUD (ICD code: F10.0 - F10.9) in 2017, 2018 and 2019 were 7.6%, 9.1% and 10%, respectively, with variability across health regions. After nationwide implementation of the alcohol service system in 2018, the alcohol screening and brief intervention was provided to 32.7% of health service users. The self-evaluation of alcohol system readiness in selected hospitals revealed that 75% of the service units, including outpatient unit, NCD clinic, mental/substance clinic, primary care unit, and emergency unit, were able to deliver the alcohol screening and brief intervention. However, alcohol related services in more than half of the hospitals (55.2%) were only performed by mental and substance specialty staff nurses.
Conclusion: The alcohol treatment policy implementation has increased the alcohol treatment accessibility. However, the human resources preparation and training for the specific knowledge and skills should be considered especially the mental health/substance specialty nurses who are maninly facilitated and support the services.
