บทคัดย่อ

วัตถุประสงค์: เพื่อสำรวจความชุกของภาวะซึมเศร้าและความคิดฆ่าตัวตายของวัยรุ่นไทยในโรงเรียนในภาพรวมของประเทศ และ 13 เขตสุขภาพ

วิธีการ: กลุ่มตัวอย่าง ได้แก่ วัยรุ่นอายุ 11-19 ปี ที่ศึกษาในโรงเรียนเป็นตัวแทน 13 จังหวัด จาก 13 เขตสุขภาพทั่วประเทศ ใช้แบบประเมินภาวะซึมเศร้าในวัยรุ่นฉบับภาษาไทย (Thai version of Patient Health Questionnaire for Adolescents; PHQ-A) วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและวิเคราะห์ความสัมพันธ์โดยใช้ Odds Ratio (OR)

ผล: กลุ่มตัวอย่าง 5,345 คน พบวัยรุ่นที่มีภาวะซึมเศร้า (คะแนน PHQ-A ≥ 10) ร้อยละ 17.5 และมีภาวะซึมเศร้าเล็กน้อย (5-9 คะแนน) ร้อยละ 49.8 โดยเขตสุขภาพที่พบวัยรุ่นมีภาวะซึมเศร้ามากที่สุด 3 อันดับแรก ได้แก่ เขตสุขภาพที่ 5 (ร้อยละ 25.0), เขตสุขภาพที่ 7 (ร้อยละ 24.0) และเขตสุขภาพที่ 13 (ร้อยละ 23.9) พบวัยรุ่นที่มีความคิดอยากตาย ร้อยละ 20.5 มีความคิดฆ่าตัวตาย ร้อยละ 5.1 และเคยพยายามฆ่าตัวตาย ร้อยละ 6.4 จากวัยรุ่นที่มีความเสี่ยงฆ่าตัวตายทั้งหมด ร้อยละ 22.5 มีวัยรุ่นที่มีความเสี่ยงสูง ร้อยละ 2.2 ทั้งนี้ วัยรุ่นที่มีความเสี่ยงฆ่าตัวตายมีโอกาสที่จะมีภาวะซึมเศร้ามากกว่าวัยรุ่นที่ไม่มีความเสี่ยงฆ่าตัวตาย 9.8 เท่า (OR = 9.8; 95% CI. 8.4-11.5)

สรุป: ผลการวิจัยนี้แสดงให้เห็นถึงขนาดของปัญหาภาวะซึมเศร้าและฆ่าตัวตายของวัยรุ่นในบริบทโรงเรียนซึ่งควรได้รับการดูแลช่วยเหลืออย่างเร่งด่วน ชี้ให้เห็นถึงความสำคัญของระบบคัดกรองด้านสุขภาพจิตในโรงเรียนและการส่งต่อสถานบริการสาธารณสุขในพื้นที่ใกล้เคียง
