Abstract

Background: Road traffic injuries (RTIs) are a significant public health problem in Thailand, particularly in Health Region 7. As a tertiary care center, Khon Kaen Hospital conducted this study to evaluate the survival rate of RTI patients, in order to assess effectiveness of care and provide data for future improvement.

Objectives: To determine the survival rate and to describe the general characteristics and clinical presentation of road traffic injury patients admitted to Khon Kaen Hospital between 2019 and 2024.

Methods: This retrospective cohort study used inpatient data from the Injury Surveillance (IS) system (ICD-10 codes V01-V89) collected between January 1, 2019 and December 31, 2024, with survival status followed until January 31, 2025. Survival was analyzed using the Kaplan-Meier method. The mortality rate was reported per 1,000 person-days, and its 95% confidence interval was calculated using a Poisson distribution

Results: Among 6,849 injured patients, with a total follow-up of 53,281 person-days, 538 deaths occurred, giving a mortality rate of 10.10 per 1000 person-days (95% CI: 9.30-11.00). Most patients were male (74.35%), with a mean age of 39.11 years (SD = 18.87 years). Motorcycles were involved in 85.57% of cases, and head/neck injury was the most common injury site (71.10%). Most patients had a Glasgow Coma Scale (GCS) of 13-15 (78.16%), and just over half had severe injuries (ISS 16-75, 50.39%). Survival rates at 1, 15, and 30 days were 96.04% (95% CI: 95.55-96.48), 89.09% (95% CI: 87.94-90.14), and 82.00% (95% CI: 79.62-84.13), respectively, showing a continuous decline in survival during the first 30 days.

Conclusions: Survival rates were high compared to the regional (health-region) level, reflecting the hospital's status as a tertiary-care center and a Level 1 Excellence Center in Trauma and Emergency care. Future studies should analyze factors associated with patient
