The development of the homebound patient and bedbound patient care system in Tambon Nong Bou Amphoe Moung Nong Bua Lamphu Province

The purposes of this participatory action research were 1) to develop care system for homebound and bedbound patients living in Nong Bou sub-district, Mueang District, Nong Bua Lamphu and 2) to monitor effectiveness of this developed care system. Thirty-five homebound and bedbound patients with Barthel ADL Index ≤11 points were included in this study. Participants’ demographic data were collected and presented by descriptive statistics. After implemented this new care system, data were analysed by dependent t-test (paired t-test). Findings showed that average age of bedbound and homebound patients was 63.3 years old (SD=22.5). The majority of bedbound patients (65.7%) were being elderly. Four main activities to develop the care system for homebound/bedbound system included developing the practical guideline, managing patient database system, setting family care team and building community partnership system. After applying these activities, Barthel ADL Index of participants were increased 2.6 points (95%CI: 1.4 – 3.6, p<0.001). In 11 patients (31.4%), complications mainly from urinary tract infections and pneumonia were decreased (p=0.005) significantly.
