The development of health care model for Diabetes Mellitus and Hypertension patients through the network participation in Rattanawapi District, Nong Khai Province

Abstract

The aim of participation action research was to study the development of health care model for Diabetes Mellitus (DM) in Rattanawapi hospital, Nong Khai province. The sample size were 13 staff of interprofessional and 60 DM patients who registered in Rattanawapi hospital who selected by purposive sampling between October 2017 and March 2018. For a qualitative research data with focus group discussion guideline. The data was analyzed by SPSS for Windows programs to acquire percentage, mean, standard deviation, minimum and maximum, and paired sample t-test were administered to determine the mean score difference of knowledge satisfaction and action. For a qualitative data was analyzed by content analysis. The finding revealed that the health care model for DM were four guidelines, including; providing patients care based on the stakeholder participation (UCCARE), improving knowledge and skills of village volunteers; giving knowledge and essential skills for patients’ self-care; and visiting patients 2 times a week. Regarding the effects of the developed model, the scores of self-care behaviors in overall increased from 76.27% to 84.22% and the controlling of DM evaluated using fasting blood sugar, HepBA1C and e- GFR, was significant (p <0.05). For the conclusion and suggestions, the development of model or the system of DM patient care need to collaborate with stakeholder under the possibility of the potential. This model should be promoted in health promoting hospital and health-related organizations, as well as the establishment of a group of DM patients or patients with chronic diseases in community should be concerned.
