แรงบีบมือกับการทำนายสมรรถภาพการออกกำลังกายในสตรีที่มีภาวะอ้วน: การศึกษาแบบย้อนหลัง

บทนำ: ภาวะโรคอ้วนเป็นปัญหาสาธารณสุขที่สำคัญระดับโลกโดยเฉพาะอย่างยิ่งในกลุ่มผู้หญิง ส่งผลกระทบต่อสุขภาพทั้งร่างกายและจิตใจ 6-Minute Walk Test (6MWT) เป็นเครื่องมือที่ใช้กันอย่างแพร่หลายในการประเมินสมรรถภาพทางกาย อย่างไรก็ตาม การทดสอบดังกล่าวอาจทำได้ยากในผู้ป่วยที่มีโรคอ้วน เนื่องจากความปลอดภัยหรือข้อจำกัดด้านพื้นที่และเวลาในการทดสอบ ในขณะที่แรงบีบมือเป็นการทดสอบที่ทำได้ง่าย รวดเร็ว ใช้อุปกรณ์ไม่ซับซ้อน และสะท้อนถึงความแข็งแรงของกล้ามเนื้อโดยรวมและความทนทานต่อการเดินระยะไกล เช่น 6MWT จุดประสงค์ของการวิจัยนี้เพื่อศึกษาความสัมพันธ์ของแรงบีบมือต่อการทดสอบ 6MWT ในผู้หญิงที่มีโรคอ้วน
วิธีการศึกษา: การศึกษานี้เป็นการศึกษาแบบย้อนหลังในผู้หญิงที่มีโรคอ้วนซึ่งมีค่าดัชนีมวลกาย (BMI) ตั้งแต่ 25 kg/m2 ขึ้นไปที่ได้รับการส่งปรึกษากายภาพบำบัด ณ หน่วยกายภาพบำบัด โรงพยาบาลศรีนครินทร์ คณะแพทยศาสตร์ มหาวิทยาลัยขอนแก่น รวมทั้งสิ้น 57 ราย ประเมินแรงบีบมือโดย Handgrip dynamometer และประเมินสมรรถภาพทางกายโดยการทดสอบการเดิน 6 นาที (6MWT) วิเคราะห์ความสัมพันธ์โดย Spearman’s correlation และประเมินความสามารถในการจำแนกผู้ที่มีความสามารถในการออกกำลังกายลดลง (6MWT < 450 m) โดย Receiver operating characteristic (ROC) analysis
ผลการศึกษา: กลุ่มตัวอย่างเป็นเพศหญิงมีค่ามัธยฐานของอายุ 31 (IQR = 18) ปี ค่ามัธยฐานของ BMI 38.3 (IQR = 9.4) kg/m2 ค่าเฉลี่ยของแรงบีบมือเท่ากับ 24.1 (SD = 6.2) กิโลกรัม และค่ามัธยฐานของ 6MWT เท่ากับ 363 (IQR = 59) เมตร แรงบีบมือมีความสัมพันธ์เชิงบวกกับ 6MWT (rs = 0.59, p < 0.001) การวิเคราะห์ ROC พบว่าแรงบีบมือสามารถจำแนกผู้ที่มีสมรรถภาพทางกายลดลงได้ดี (AUC [95% CI] = 0.75 (0.55 - 0.96) โดยมีค่าจุดตัด 27 กิโลกรัม โดยมีค่า Sensitivity 80% และ Specificity 69.2%
สรุป: แรงบีบมือมีความสัมพันธ์กับสมรรถภาพทางกายในผู้หญิงที่มีโรคอ้วนและอาจใช้เป็นเครื่องมือคัดกรองสมรรถภาพทางกายลดลงในทางคลินิกได้
