Factor Related to Pre-hospital Delay in Acute Ischemic Stroke Patients at Somdejprabuddhalertla Hospital

Abstract

Background and objective: Treatment of patients with acute ischemic stroke (AIS) is time related, delay in treatment may warrant adverse prognosis. Study aimed to explore the pre-hospitalization delay related factors

Methods: Between March to August 2021, a cross-sectional study was carried out in 72 patients admitted with AIS at Somdejprabuddhalertla hospital. All patients were interviewed with a questionnaire focused on socio-demographic, clinical, situational, type of transfer, the stroke awareness, the perception of stroke severity and the response to stroke symptoms. Data were analyzed to identify the factors influencing pre-hospital delay [> 4.5 hours after stroke onset].

Results: 72 patients were enrolled in the study, 58.3% patients arrived after 4.5 hours of onset, gradual onset (OR 5.45,95%CI 2.42-16.6), awakening with symptoms (OR 2.8, 95%CI0.57-13.55), doing nothing and waiting for recovery by yourself (OR 82,95%CI 6.39-157.18 ), no helper at onset (OR 4.49, 95%CI 0.42-4.57), not recognizing the symptom as stroke(OR 4.88, 95%CI 0.59-4.04), not recognizing the severity as stroke(OR2.14,95%CI 0.29- 19.15) had  a significantly more delay in arriving to the hospital.

Conclusion: The findings indicate that health care professionals should provide more information about stroke knowledge, awareness, and perception of stroke severity that importance of cognitive factors on the decision-making process and pre-hospital delays.
