The Effective of Pharmacist Counseling to Patients Receiving Warfarin at Bangsaphan Hospital, Prachuap Kirikhan

Abstract

This descriptive study aimed to assess the outcomes of pharmacist’ s counseling on outpatients receiving warfarin therapy at Bangsaphan hospital. This study design to compared patient’ s knowledge and International Normalized Ratio ( INR) , present the data by descriptive statistic such as mean, SD, percent and the referal statistic such as paired t-test and chi square test. The study participants were out- patients receiving warfarin since October, 2016. And had been followed up at warfarin clinic for 6 months. Patient’ s knowledge on warfarin was tested before and after 6 months. Results: 95 patients, their mean age was 60 years, the majority was female 57.90% with having warfarin for atrial fibrillation 50. 53% , and 30. 53% having an INR in therapeutic range. Knowledge score on warfarin was increased significantly after counseling (2.63% to 8.83%). Frequencies of INR within therapeutic range post- counseling were higher than pre- counseling ( 30. 53% to 54.73%). Conclusion that patient counseling on warfarin by pharmacist improves patients’ knowledge and clinical outcomes.
