ผลลัพธ์ของระบบติดตามสุขภาพทางไกลต่อการควบคุมระดับน้ำตาลและผลลัพธ์ที่รายงานโดยผู้ป่วยในผู้ป่วยเบาหวานชนิดที่ 2 ที่ควบคุมระดับน้ำตาลได้ไม่ดี : แนวทางการดูแลสุขภาพโดยเน้นคุณค่าที่โรงพยาบาลเวียงป่าเป้า จังหวัดเชียงราย

ความสำคัญ: โรคเบาหวานชนิดที่2 เป็นปัญหาสาธารณสุขที่สำคัญ ผู้ป่วยจำนวนมากไม่สามารถควบคุมระดับน้ำตาลในเลือดได้ ทำให้เกิดภาวะแทรกซ้อนที่รุนแรงตามมา การนำระบบติดตามสุขภาพทางไกล (Telehealth monitoring) มาใช้ติดตามผู้ป่วยร่วมกับการรักษาตามมาตรฐาน เป็นแนวทางใหม่ที่อาจช่วยเพิ่มประสิทธิผลการดูแลผู้ป่วยและสอดคล้องกับแนวคิด Value-based health care
วัตถุประสงค์: เพื่อศึกษาผลลัพธ์ของการใช้ระบบติดตามสุขภาพทางไกลต่อการควบคุมระดับ HbA1c, FBSและผลลัพธ์ ที่รายงานโดยผู้ป่วย (PROMs,PREMs) ในผู้ป่วยเบาหวานชนิดที่ 2 ที่ควบคุมระดับน้ำตาลได้ไม่ดี
วิธีการศึกษา: การศึกษานี้เป็นแบบ quasi-experimental ศึกษาผู้ป่วยเบาหวานชนิดที่ 2 ของโรงพยาบาลเวียงป่าเป้า จำนวน 128 ราย แบ่งเป็นกลุ่มทดลองที่ได้รับการติดตามสุขภาพผ่านแอปพลิเคชั่นบนสมาร์ทโฟนที่มีมาตรฐาน PDPA ร่วมกับการรักษาตามมาตรฐาน (n=64) และกลุ่มควบคุมที่ได้รับการรักษาตามมาตรฐานเพียงอย่างเดียว (n=64) ประเมินผลลัพธ์ทางคลินิก FBS, HbA1c, ภาวะแทรกซ้อน,พฤติกรรมการดูแลตนเอง,คุณภาพชีวิตและสุขภาพจิต PROMs (EQ-5D, WHO-5),ประสบการณ์ของผู้ป่วยต่อระบบติดตามสุขภาพ PREMs (TUQ, NPS) และผลทางเศรษฐศาสตร์สุขภาพ ใช้สถิติ t-test และ Poisson regression
ผล: ผู้ป่วยเบาหวานทั้งสองกลุ่มมีลักษณะพื้นฐานได้แก่ เพศ อายุ ดัชนีมวลกาย โรคร่วม พฤติกรรมการดูแลตนเอง FBS และ HbA1c เริ่มต้นไม่แตกต่างกัน ภายหลังการทดลอง 6 เดือน พบว่ากลุ่มทดลองมีแนวโน้มการเปลี่ยนแปลงของ FBS ลดลงมากกว่ากลุ่มควบคุมทั้งในเดือนที่ 2, 4 และ6 หลังการทดลองคือ 121 ± 33.4 VS 146 ± 30.6, (p<0.001) HbA1c ในเดือนที่ 6 หลังการทดลอง กลุ่มทดลองมีค่า HbA1c ต่ำกว่ากลุ่มควบคุม คือ 7.34 ± 1.13% VS 8.59 ± 0.98% (p<0.001) ภาวะแทรกซ้อนระหว่างการรักษาของทั้งสองกลุ่มไม่แตกต่างกัน ผลด้านการควบคุมอาหารอย่างสม่ำเสมอของกลุ่มทดลองสูงกว่ากลุ่มทดลองอย่างมีนัยสำคัญทางสถิติในเดือนที่2,4 และเดือนที่ 6 ผลด้านการออกกำลังกายของทั้งสองกลุ่มไม่แตกต่างกัน ผลด้านการรับประทานยาของทั้งสองกลุ่มไม่แตกต่างกัน
ผลด้านคุณภาพชีวิตและสุขภาพจิตของทั้งสองกลุ่มไม่แตกต่างกัน ผลด้านประสบการณ์ของผู้ป่วยต่อระบบติดตามสุขภาพแอปพลิเคชั่นใช้ง่ายมาก 85.90% แอปพลิเคชั่นช่วยให้การบริหารโรคเบาหวานดีขึ้น93.75% จำนวนครั้งและค่าใช้จ่ายเฉลี่ยของการเข้ารักษาในโรงพยาบาลและการเข้ารับบริการที่คลินิกเบาหวานในระยะเวลา 6 เดือน ไม่แตกต่างกัน
ข้อสรุป และข้อเสนอแนะ: ระบบ Telehealth monitoring สามารถเพิ่มประสิทธิภาพในการควบคุมระดับน้ำตาล เพิ่มความสม่ำเสมอของการควบคุมอาหาร ใช้ง่าย และช่วยในบริหารโรคเบาหวานดีขึ้นเพิ่มความสะดวกในการประสานงาน และสะท้อนถึงแนวทางการดูแลสุขภาพที่เน้นคุณค่าในโรงพยาบาลชุมชนโดยใช้เทคโนโลยี
