บทคัดย่อ
ความเป็นมา: ภาวะเบาหวานเข้าจอประสาทตา (diabetic retinopathy; DR) เป็นภาวะแทรกซ้อนที่สําคัญของผู้ป่วยเบาหวาน ทําให้เกิดการสูญเสียการมองเห็นได้ การรักษาโรคเบาหวานอย่างเหมาะสม ร่วมกับการตรวจคัดกรองภาวะเบาหวานเข้าจอประสาทตาอย่างสม่ําเสมอ ช่วยป้องกันภาวะดังกล่าวได้
วัตถุประสงค์: เพื่อศึกษาความชุกและปัจจัยที่เกี่ยวข้องกับภาวะเบาหวานเข้าจอประสาทตาในผู้ป่วยเบาหวานชนิดที่ 2 ในหน่วยบริการปฐมภูมิ โรงพยาบาลเชียงรายประชานุเคราะห์ ด้วยเครื่องตรวจปัญญาประดิษฐ์ (AI)
วิธีการศึกษา: เป็นการศึกษาย้อนหลังแบบตัดขวาง ผู้ป่วยเบาหวานชนิดที่ 2 ที่เข้ารับการรักษาในห้องตรวจโรคไม่ติดต่อเรื้อรัง และได้รับการตรวจคัดกรองภาวะเบาหวานเข้าจอประสาทตา ด้วยเครื่องตรวจปัญญาประดิษฐ์ ณ หน่วยบริการปฐมภูมิ โรงพยาบาลเชียงรายประชานุเคราะห์ ตั้งแต่วันที่ 1 มกราคม ถึง 31 ธันวาคม พ.ศ. 2567 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและวิเคราะห์ถดถอยแบบลอจิสติก
ผลการศึกษา: ผู้ป่วย 971 ราย พบความชุกของภาวะเบาหวานเข้าจอประสาทตา ร้อยละ 43.7 แบ่งเป็น moderate non-proliferative DR (NPDR) ร้อยละ 29.3 mild NPDR ร้อยละ 12.2 severe NPDR ร้อยละ 1.6 และ proliferative DR (PDR) ร้อยละ 0.7 ปัจจัยที่สัมพันธ์กับภาวะเบาหวานเข้าจอประสาทตาอย่างมีนัยสําคัญทางสถิติ ได้แก่ ระยะเวลาที่ป่วยเป็นเบาหวานมากกว่า 10 ปี (aOR 1.78, 95% CI: 1.36-2.33, p<0.001) โรคไตวายเรื้อรังตั้งแต่ระยะที่ 3 (aOR 1.42, 95% CI: 1.05-1.91, p=0.021) ระดับน้ําตาลสะสม (HbA1c) ≥7% (aOR 1.50, 95% CI: 1.14-1.96, p=0.003) และระดับไขมัน LDL ≥100 mg/dl (aOR 0.66, 95% CI: 0.50-0.87, p=0.004)
สรุปและข้อเสนอแนะ: พบความชุกภาวะเบาหวานเข้าจอประสาทตาด้วยเครื่องตรวจปัญญาประดิษฐ์ ร้อยละ 43.7 ซึ่งมากกว่าการศึกษาก่อนหน้า เนื่องจากการคัดกรองด้วย AI มีความไวและความจําเพาะสูงในการวินิจฉัยภาวะเบาหวานเข้าจอประสาทตาระยะเริ่มต้นได้มากขึ้น ดังนั้นผู้ป่วยที่เป็นเบาหวานระยะเวลามากกว่า 10 ปี โรคไตวายเรื้อรังตั้งแต่ระยะ 3 และระดับ HbA1c ≥7% ควรตะหนักและเฝ้าระวังในกลุ่มเสี่ยงนี้ เพื่อนําไปสู่การวินิจฉัยภาวะเบาหวานเข้าจอประสาทตาและให้ได้รับการรักษาที่เหมาะสม ลดการเกิดการสูญเสียการมองเห็นในอนาคต
