บทคัดย่อ
ความเป็นมา: ความต้องการด้านจิตวิญญาณสะท้อนถึงความต้องการในการค้นหาความหมายและเป้าหมายของชีวิต การมีภาวะจิตวิญญาณที่ดีจะช่วยให้ผู้ป่วยสามารถเผชิญหน้าและปรับตัวต่อภาวะวิกฤติจากการเจ็บป่วยได้อย่างเหมาะสม แต่ในทางปฏิบัติ การประเมินยังไม่ครอบคลุมและขาดแนวทางที่ชัดเจน งานวิจัยครั้งนี้จึงมุ่งศึกษาถึงความชุกรวมถึงปัจจัยที่สัมพันธ์กับความต้องการด้านวิญวิญญาณระดับสูงของผู้ป่วยมะเร็งที่รักษาประดับประคองแบบผู้ป่วยในของโรงพยาบาลเชียงรายประชานุเคราะห์ ซึ่งเป็นโรงพยาบาลระดับตติยภูมิ เพื่อเป็นแนวทางให้ประเมินได้อย่างเหมาะสมและการให้ดูแลได้อย่างรอบด้าน
วัตถุประสงค์: เพื่อศึกษาความชุกและปัจจัยที่สัมพันธ์กับความต้องการด้านจิตวิญญาณระดับสูง ของผู้ป่วยมะเร็งที่รักษาประคับประคองแบบผู้ป่วยใน ณ โรงพยาบาลเชียงรายประชานุเคราะห์
วิธีการศึกษา: ศึกษาแบบ cross-section study ใช้แบบสอบถามประเมินระดับความต้องการและปัจจัยที่สัมพันธ์กับความต้องการด้านจิตวิญญาณ เก็บข้อมูลในผู้ป่วยมะเร็ง 100 คน ตั้งแต่วันที่ 1 พฤศจิกายน พ.ศ. 2567 ถึง 31 สิงหาคม พ.ศ. 2568 ศึกษาความชุกและความสัมพันธ์กับความต้องการด้านจิตวิญญาณระดับสูง โดยใช้วิธี multivariable logistic regression กำหนดนัยสำคัญทางสถิติโดยใช้ค่า p<0.05
ผลการศึกษา: พบผู้ป่วยที่มีความต้องการด้านจิตวิญญาณระดับสูง จำนวน 17 คน (ร้อยละ 17) โดยปัจจัยที่สัมพันธ์อย่างมีนัยสำคัญ ได้แก่ การยังคงประกอบอาชีพ (aOR 5.55, 95% CI: 1.57-19.60, p=0.008) ระดับ palliative performance status (PPS) 50-69% (aOR 6.86, 95% CI: 1.27-37.16, p=0.025) ระดับ PPS 30-49% (aOR 22.08, 95% CI: 2.51-194.21, p=0.005) และการไม่มี advance care planning (ACP) (aOR 5.30, 95% CI: 1.19-23.56, p=0.029)
สรุปและข้อเสนอแนะ: ความชุกของความต้องการด้านจิตวิญญาณระดับสูงในผู้ป่วยมะเร็งที่ได้รับการรักษาประคับประคองแบบผู้ป่วยในอยู่ที่ร้อยละ 17 โดยการยังคงประกอบอาชีพ ระดับ PPS ที่ลดลง และการไม่มี ACP มีความสัมพันธ์กับความต้องการด้านจิตวิญญาณระดับสูง ผู้ป่วยที่มีลักษณะดังกล่าว ควรได้รับการคัดกรองและการสนับสนุนด้านจิตวิญญาณอย่างเหมาะสม เพื่อเสริมสร้างความมั่นคงทางจิตใจก่อนเผชิญกับความเปราะบางในระยะท้ายของชีวิต
