The Relationship Between Transition Conditions and Outcome Indicators in Transitioning to the Position of Head Nurse at University Hospital

Introduction: Transitioning into the role of head nurse significantly impacts various aspects, leading to heightened anxiety and stress due to increased responsibilities. Transition theory outlines the favorable conditions crucial for successful adaptation to this new role.
Research Objectives: This study aimed to investigate the relationship between transition conditions and outcome indicators in transitioning to the position of head nurses at a university hospital.
Research methodology: A cross-sectional survey was conducted among head nurses from ten university hospitals with no more than 500 beds. A multistage random sampling method was employed to select a sample group of 110 individuals. The content validity index (CVI) was assessed by developing a questionnaire reviewed by three experts, resulting in a CVI of 0.84. The questionnaire was piloted with 30 hospital head nurses possessing qualifications similar to those of the sample group. It comprised three parts: 1) general information, 2) transition condition factors, and 3) outcome indicators in transitioning into the position. The questionnaire's reliability was evaluated using Cronbach’s alpha coefficient, yielding coefficients of .92 and .98, respectively.
Results: (1) Overall, the condition factors in transitioning to the head nurse position were significantly high. 2) The outcome indicators in transitioning to the head nurse position were also at a high level. 3) There was a positively statistically significant correlation between condition factors and outcome indicators in transitioning to a leadership position (p < .01).
Conclusion: Condition factors play a crucial role in transitioning to the head nurse position. High levels of these conditions contribute favorably to the transition, aligning with Meleis’ theory. Transition conditions include giving meaning, culture, beliefs, and attitudes, social and economic status, as well as preparation and knowledge. These conditions facilitate a smoother and more successful transitions.
Implications: The findings of this study can serve as guidelines for planning promotions, providing support, and preparing for transition to the position of head nurses. This will help individuals to fully adapt to their new roles as head nurses.
