Abstract

Introduction: Chronic kidney disease (CKD) is a significant global public health challenge. Providing educational interventions and home visits by interdisciplinary healthcare teams can enable patients and their caregivers to better manage the disease and reduce the risk of complications.

Research objectives: To develop a model for end-stage CKD patient care at home in Phonphisai District, Nong Khai Province, and to evaluate the effectiveness of the developed model.

Research methodology: This study used an action research approach. The study sample comprised 42 participants, including end-stage CKD patients not undergoing renal replacement therapy, their caregivers, and interdisciplinary healthcare teams, selected through a purposive sampling method. Data were collected using a set of questionnaires, focus group discussions, and in-depth interviews, conducted before and after the implementation of the home care model. Quantitative data analysis involved descriptive statistics, frequencies, means, and standard deviations, while qualitative data were analyzed using content analysis.        

Results: Main results of the study are as follows: 1) The situational analysis revealed that end-stage CKD patients and caregivers lacked adequate knowledge and skills for self-care, leading to complications. The developed home care model incorporated educational sessions and skills training for patients and caregivers, as well as guidelines for interdisciplinary home visits. 2) Following the intervention, patients demonstrated improved knowledge and skills in self-care practices at home and were better equipped to manage basic complications.         

Conclusion: The home-based care model developed in this study can be effectively implemented for end-stage CKD patients receiving care at home.

Implications: Findings of the study can inform the development of care models for CKD patients who opt for conservative management over renal replacement therapy, within the context of renal clinics.
