บทคัดย่อ

บทนำ: ผู้ป่วยโรคไตวายเรื้อรังเป็นปัญหาสาธารณสุขทั่วโลก การให้ความรู้และการติดตามเยี่ยมบ้านของทีมสหวิชาชีพ จะทำให้ผู้ป่วยและญาติมีพฤติกรรมการควบคุมโรคดีและลดภาวะแทรกซ้อน

วัตถุประสงค์การวิจัย: พัฒนารูปแบบการดูแลผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่บ้าน อำเภอโพนพิสัย จังหวัดหนองคาย และ ประเมินประสิทธิผลของการพัฒนารูปแบบ

ระเบียบการวิจัย: วิจัยเชิงปฏิบัติการ กลุ่มตัวอย่างคือ ผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่ไม่รับการบำบัดทดแทนไต ผู้ดูแล ทีมสหวิชาชีพโดยเลือกแบบเจาะจง จำนวน 42 คน เก็บข้อมูลโดยใช้ชุดแบบสอบถาม การสนทนากลุ่ม และสัมภาษณ์เชิงลึก เก็บรวบรวมข้อมูลก่อนและหลังการใช้รูปแบบการดูแลผู้ป่วยฯ วิเคราะห์ข้อมูลเชิงปริมาณโดยแจกแจงหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และข้อมูลเชิงคุณภาพโดยวิเคราะห์เชิงเนื้อหา

ผลการวิจัย: 1) สถานการณ์และปัญหาการดูแลผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่บ้าน พบว่า ผู้ป่วยและผู้ดูแลขาดความรู้และทักษะในการดูแลตนเองและเกิดภาวะแทรกซ้อน และรูปแบบการดูแลผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่บ้านที่พัฒนาขึ้น มีการให้ความรู้และฝึกทักษะให้ผู้ป่วยและผู้ดูแล แนวทางการเยี่ยมบ้านทีมสหวิชาชีพ และ 2) หลังการใช้รูปแบบฯ ผู้ป่วยมีความรู้และทักษะในการปฏิบัติตัวดูแลตนเองที่บ้านเพิ่มขึ้น และจัดการกับอาการแทรกซ้อนเบื้องต้นที่บ้านได้

สรุปผล: แนวปฏิบัติที่พัฒนานี้สามารถนำไปปฏิบัติกับผู้ป่วยโรคไตวายเรื้อรังระยะสุดท้ายที่บ้านได้

ข้อเสนอแนะ: สามารถนำผลการวิจัยไปใช้ในการพัฒนาการดูแลผู้ป่วยที่คลินิกไตเรื้อรังที่เลือกรักษาบำบัดแทนแทนทางไต
