โปรแกรมพัฒนาผู้ดูแลเด็กสมองพิการ: การทบทวนวรรณกรรม

การทบทวนวรรณกรรมอย่างเป็นระบบนี้มีวัตถุประสงค์เพื่อสังเคราะห์ลักษณะงานวิจัยและรูปแบบของโปรแกรมที่ออกแบบเพื่อพัฒนาผู้ดูแลเด็กสมองพิการ โดยสืบค้นงานวิจัยภาษาไทยและภาษาอังกฤษที่ตีพิมพ์ในฐานข้อมูลอิเล็กโทรนิกส์ ได้แก่ PubMed, CINAHL, Cochrane Library, EMBASE, Scopus, ThaiJO และ ThaiLIS ในช่วง 10 ปีที่ผ่านมา (พ.ศ. 2553 – 2562) เกณฑ์การคัดเลือกงานวิจัยเข้า ได้แก่ 1) เป็นงานวิจัยปฐมภูมิที่ศึกษาในผู้ดูแลเด็กสมองพิการ 2) เป็นงานวิจัยเชิงทดลอง กึ่งทดลอง หรืองานวิจัยปฏิบัติการอย่างมีส่วนร่วมที่ออกแบบสำหรับพัฒนาความสามารถของผู้ดูแลหลักของเด็กสมองพิการ อายุ 0-15 ปี 3) มีการประเมินผลลัพธ์ในผู้ดูแลหลักที่สอดคล้องกับโปรแกรม/กิจกรรมที่จัดให้ ผลการทบทวนวรรณกรรมมีงานวิจัยที่ผ่านเกณฑ์การคัดเลือกงานวิจัยเข้า จำนวน 8 เรื่อง สรุปผลการวิเคราะห์ได้ดังนี้
- โปรแกรมถูกออกแบบสำหรับพัฒนาความรู้ความสามารถของผู้ดูแล 3 เรื่อง (การให้อาหาร และการส่งเสริมความแข็งแรงของกล้ามเนื้อ) โปรแกรมสร้างเสริมพลังอำนาจผู้ดูแล 2 เรื่อง และ โปรแกรมส่งเสริมสุขภาพจิตและคุณภาพชีวิตผู้ดูแล 3 เรื่อง ส่วนใหญ่เป็นงานวิจัยกึ่งทดลอง ศึกษาในโรงพยาบาลหรือคลินิกฟื้นฟูสภาพ/กระตุ้นพัฒนาการ มีจำนวนกลุ่มตัวอย่างน้อยกว่า 30 คน/เรื่อง
- กิจกรรมของโปรแกรมพัฒนาผู้ดูแลเด็กสมองพิการแต่ละเรื่อง ประกอบด้วยกิจกรรมที่หลากหลาย ได้แก่ การสอนสุขศึกษา การจัดกิจกรรมกลุ่ม การให้คำแนะนำ การสาธิตและการฝึกทักษะ การเยี่ยมบ้าน การโทรศัพท์เยี่ยม การสนับสนุนสื่อ อุปกรณ์ และค่าเดินทาง ระยะเวลาการจัดกิจกรรม ส่วนใหญ่ใช้ระยะเวลา 2 เดือน (3 สัปดาห์ ถึง 2.5 เดือน) หรือ 6-8 ครั้ง
- ผลลัพธ์ของโปรแกรม สรุปได้ว่า 1) การจัดกิจกรรมที่หลากหลาย เป็นระยะเวลา 2 เดือน หรือ 6-8 ครั้ง เพียงพอที่จะทำให้เกิดการเปลี่ยนแปลงของพฤติกรรม/ทักษะ/ความสามารถในการดูแลเด็กสมองพิการของผู้ดูแล 2) การให้สุขศึกษาและการให้คู่มือ เป็นระยะเวลา 8 สัปดาห์ติดต่อกัน ไม่มีประสิทธิผลทำให้เกิดการเปลี่ยนแปลงของความเครียดและคุณภาพชีวิตของผู้ดูแล 3) โปรแกรมที่มีกิจกรรมที่หลากหลายและมีการติดตามต่อเนื่อง เช่น การเยี่ยมบ้าน หรือการโทรเยี่ยม รวมอยู่ด้วย ทำให้เกิดผลลัพธ์ที่ดีในตัวแปรที่ศึกษา
สรุปว่าควรมีโปรแกรมพัฒนาผู้ดูแลเด็กสมองพิการเพิ่มมากขึ้นโดยเฉพาะในบริบทของชุมชน และโปรแกรมพัฒนาผู้ดูแลควรประกอบด้วยกิจกรรมที่หลากหลายและมีการติดตามต่อเนื่องเพื่อนำไปสู่ผลลัพธ์ที่ดี
