รูปแบบการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว

บทคัดย่อ

            การวิจัยนี้เป็นการวิจัยเชิงอนาคตโดยมีวัตถุประสงค์เพื่อสังเคราะห์และประเมินรูปแบบการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว ดำเนินการวิจัยโดยการศึกษาภาคสนาม เก็บรวบรวมข้อมูลโดยการสนทนากลุ่มจากกลุ่มตัวอย่างที่เป็นผู้นำภาคีเครือข่ายการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว จำนวน 6 อำเภอ ๆ ละ 10 คน รวม 60 คน และแบบสัมภาษณ์เชิงลึก จากผู้เชี่ยวชาญ 5 สาขา จำนวน 17 คน ระหว่างวันที่ 1 พฤศจิกายน 2560-28 กุมภาพันธ์ 2561 และวิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา (Content analysis)

             ผลการวิจัย พบว่า รูปแบบการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว สามารถจัดเป็นระบบ (Systematic) โดยมีปัจจัยแห่งความสำเร็จหลัก 6 องค์ประกอบสำคัญที่สัมพันธ์เชื่อมโยงกันอย่างเป็นระบบ ได้แก่ ปัจจัยนำเข้า 1 องค์ประกอบ คือ 1) การทำงานร่วมกันในระดับอำเภออย่างเข้มแข็ง (Unity of District Health Team:U)  เป็นโอกาสที่มีระเบียบสนับสนุนให้การทำงานเป็นทางการชัดเจนขึ้น กระบวนการ 4 องค์ประกอบ คือ 2) การมีส่วนร่วมของเครือข่ายและชุมชน (Community participation: C) ที่ทำงานด้วยหัวใจที่ “ระเบิดจากข้างใน”จนมีความตระหนักและรู้สึกเป็นเจ้าของ 3) การแบ่งปันทรัพยากรและการพัฒนาบุคลากร (Resource sharing and human development: R) แบบ “พี่มีสองน้องมีหนึ่ง” พึ่งพากันด้วยแรงศรัทธาสามัคคี 4) การชื่นชมการทำงานและการเรียนรู้ร่วมกัน (Appreciation Inquiry: A) จากการปฏิบัติแบบ “ฉันท์พี่น้อง” 5) การให้บริการสุขภาพตามความจำเป็นในบริบทของชุมชน (Essential Health care: E) ร่วมกันดูแลผู้อยู่ในภาวะเปราะบางด้วยความเคารพศักดิ์ศรีคุณค่าความเป็นมนุษย์ และ ผลลัพธ์ 1 องค์ประกอบ คือ 6) การมุ่งผลลัพธ์ที่เกิดกับผู้บริโภค (Customer focus: C) โดยให้ประชาชนร่วมมือปฏิบัติทุกขั้นตอนได้รับรู้ข้อมูลด้วยตนเองจึงเป็นพลังให้เกิดการมีส่วนร่วมอย่างเข้มแข็งจริงจัง ดังนั้น จึงพบว่า ระบบสุขภาพอำเภอชายแดนไทย-ลาว ที่พึงประสงค์ในการจัดการสุขภาพสำหรับประชาชนประกอบด้วย 10 องค์ประกอบหลัก 28 องค์ประกอบรอง และ 75 ประเด็นสำคัญ ที่ผู้เชี่ยวชาญมีความคิดเห็นสอดคล้องกัน (Consensus) ว่ามีความเหมาะสม ร้อยละ 80.00  มีความเป็นไปได้ในระดับมาก (ค่าฐานนิยมระดับ 4) ขึ้นไป และค่าพิสัยระหว่างควอไทล์ (Inter-quartile range) ที่ไม่เกิน 1.00 จึงมีความเหมาะสมและมีความเป็นไปได้ในการนำไปสู่การปฏิบัติจริง และปัจจัยเสริมสู่ความสำเร็จอีก 4 องค์ประกอบ คือ 7) การทำหน้าที่ของหัวหน้าทีม (Captaincy: C)  8) การสื่อสารที่มีประสิทธิภาพระหว่างภาคีเครือข่าย (Communication: C)  9) ความมุ่งมั่นในข้อตกลงร่วมกันของภาคีเครือข่าย (Commitment: C) และ 10) วัฒนธรรมความสัมพันธ์แบบเครือญาติ (Culture: C) = CCCC: 4C  ซึ่งระบบสุขภาพอำเภอชายแดนไทย-ลาว ที่พึงประสงค์ ซึ่งสามารถเขียนสรุปองค์ประกอบรูปแบบการพัฒนาระบบสุขภาพอำเภอชายแดนไทย-ลาว ได้ดังนี้ DHS@Thai-Laos’ border : 4C+U+CARE+C ข้อเสนอแนะสำหรับหน่วยงานสาธารณสุขระดับนโยบาย ควรสนับสนุนให้นำแนวคิด 4C+U+CARE+C ไปปรับใช้ในการพัฒนาระบบสุขภาพอำเภอตามบริบทของพื้นที่ชายแดน และในพื้นที่อำเภอปกติ เพื่อเป็นการพัฒนารูปแบบที่เหมาะสมและมีประสิทธิภาพยิ่งขึ้น และเป็นการยกระดับระบบสุขภาพอำเภอต่อไป
