การส่งเสริมการกลืนอย่างปลอดภัยในผู้ป่วยโรคหลอดเลือดสมอง ที่มีภาวะกลืนลำบาก: บทบาทที่ท้าทายสำหรับพยาบาล

บทคัดย่อ

          ภาวะกลืนลำบากเป็นภาวะที่พบบ่อยในผู้ป่วยโรคหลอดเลือดสมอง และเป็นปัจจัยเสี่ยงที่ทำให้เกิดปอดอักเสบ ผู้ป่วยฟื้นตัวช้า รวมถึงการเสียชีวิตในที่สุด บทบาทสำคัญของพยาบาลในการดูแลผู้ป่วยโรคหลอดเลือดสมองไม่ให้เกิดภาวะแทรกซ้อนจากภาวะกลืนลำบาก จำเป็นอย่างยิ่งที่พยาบาลจะต้องมีการส่งเสริมการกลืนตามหลักวิธีอย่างถูกต้อง เริ่มจากการประเมินความพร้อมของผู้ป่วยก่อนเริ่มการกลืน ซึ่งเป็นขั้นตอนแรกของกระบวนการพยาบาลที่สำคัญ ทำให้ผู้ป่วยสามารถกลืนอาหารและนํ้าได้อย่างปลอดภัย ประกอบด้วย 1) การประเมินระดับความรู้สึกตัว 2) การประเมินความสามารถในการทำตามคำบอก 3) การจัดท่านั่งตัวตรงหรือศีรษะสูง 90 องศา 4) การทดสอบการกลืน และ 5) การสังเกตอาการผิดปกติที่อาจเกิดขึ้น จากการทดสอบการกลืน เมื่อผู้ป่วยผ่านการประเมินความพร้อมก่อนเริ่มการกลืนทั้ง 5 ข้อแล้วจะให้การพยาบาลในเรื่อง 1) การดูแลความสะอาดปากฟัน 2) การฝึกบริหารกล้ามเนื้อการกลืน 3) การฝึกการกลืนและการประเมินความพร้อมของผู้ป่วย 4) การจัดสิ่งแวดล้อม 5) วิธีป้อนอาหาร และ 6) การประเมินผลขณะดูแลการรับประทานอาหารของผู้ป่วย เพื่อนำไปสู่การกลืนอย่างปลอดภัยในผู้ป่วยโรคหลอดเลือดสมองที่มีภาวะกลืนลำบาก ซึ่งเป็นบทบาทที่ท้าทายสำหรับพยาบาลเป็นอย่างยิ่ง
