Abstract

Abstract

Postpartum hemorrhage (PPH) is one of the major causes of maternal morbidity and mortality worldwide. This study aimed to develop the clinical nursing practice guideline (CNPG) for prevention of postpartum hemorrhage in the first two hours after giving birth. The IOWA Model of evidence-base practice to promotes quality care (2001) was used as a framework for developing the CNPG. This study was divided into 3 phases. The first phase consisted of 4 steps: 1) analysis of the problems encountered in the hospital 2) appointment of interdisciplinary CNPG developer team 3) arrange a meeting to review empirical evidences, and 4) development of the CNPG in conjunction with an interdisciplinary team and verified by experts. The new developed CNPG included 4 sections:1) attended stage, 2) delivery stage, 3) placenta delivery stage, and 4) postpartum period in the first two hours after giving birth and evaluation and preparation of maternal before moving to the postpartum ward. The second phase, CNPG was applied into practice during March-April 2017 in 30 mothers with normal labor and 22 nurses who used the CNPG for prevention of postpartum hemorrhage in the first two hours after giving birth. The third phase of CNPG outcome evaluation found that: 1) after applying the CNPG in 30 mothers with normal labor, there was no postpartum hemorrhage occurred during the first two hours after delivery; 2) the professional nurses perceived feasibility and benefit of applying the CNPG for prevention of Postpartum Hemorrhage in the first two hours after giving birth at all stages; 3) The top three opinions on the CNPG rated by the professional nurses were 1) can be used (mean =4.73, SD=0.456), 2) useful for nursing (mean=4.59, SD=0.503), and 3) satisfaction to the CNPG (mean=4.50, SD=0.512). The result indicated that the CNPG for prevention of postpartum hemorrhage in the first two hours after delivery is effective for preventing postpartum hemorrhage. Thus, the guidelines should be integrated as a part of continuous quality improvement (CQI) in order to improve quality of nursing care.
