การพัฒนาระบบการป้องกันการติดเชื้อทางเดินหายใจส่วนล่างแบบบูรณาการในผู้ป่วยมะเร็งศีรษะและคอ

บทคัดย่อ
การติดเชื้อทางเดินหายใจส่วนล่างเป็นภาวะแทรกซ้อนสำคัญในผู้ป่วยมะเร็งศีรษะและคอ เนื่องจากภาวะกลืนลำบากและเสี่ยงสำลักร่วมกับภูมิคุ้มกันลดลง ส่งผลให้เกิดการรักษาล่าช้า ระยะเวลานอนโรงพยาบาลเพิ่มขึ้น และเกิดผลลัพธ์ไม่พึงประสงค์ งานวิจัยนี้มุ่งพัฒนา “ระบบการป้องกันการติดเชื้อทางเดินหายใจส่วนล่างแบบบูรณาการ” ที่เชื่อมโยงการเฝ้าระวังเชิงรุก การปฏิบัติการพยาบาลแบบ bundle และการส่งเสริมพฤติกรรมสุขภาพของผู้ป่วย โดยดำเนินการวิจัยเชิงปฏิบัติการ (Action Research) ดำเนินการระหว่างเดือนมกราคม–ธันวาคม 2566 แบ่ง 4 ระยะ ได้แก่ (1) วิเคราะห์สถานการณ์และความต้องการของผู้ปฏิบัติงาน โดยประชุมกลุ่ม/สัมภาษณ์ ICN, ICWN และหัวหน้าหอผู้ป่วยรวม 11 ราย และทบทวนวรรณกรรม (2) พัฒนารูปแบบเฝ้าระวังการติดเชื้อแบบออนไลน์ร่วมกันระหว่าง ICN และ ICWN (3) พัฒนาระบบป้องกันการติดเชื้อฯ และนำไปใช้ ประกอบด้วยการประเมินความเสี่ยงต่อการสำลัก/ความสามารถในการกลืน/สุขภาพช่องปาก/ความเสี่ยงปอดอักเสบ ร่วมกับคู่มือการพยาบาล การฝึกการกลืน การดูแลช่องปาก และ bundle of care (4) ประเมินผลลัพธ์ กลุ่มตัวอย่างเป็นผู้ป่วยมะเร็งศีรษะและคอเสี่ยงสูง 60 ราย แบ่งออกเป็น 2 กลุ่ม ๆ ละ 30 ราย การวิเคราะห์โดยใช้สถิติเชิงพรรณนา เปรียบเทียบความแตกต่างด้วยการทดสอบทีกลุ่มอิสระ และไคสแควร์
ผลการวิจัยพบว่า (1) ระบบเฝ้าระวังแบบออนไลน์มีประสิทธิภาพสูงกว่าระบบเดิมอย่างมีนัยสำคัญ (p = .044) และพบการติดเชื้อส่วนใหญ่เป็นปอดอักเสบ (50%) และการติดเชื้อทางเดินหายใจส่วนล่าง (20%) (2) กลุ่มทดลองมีอัตราการติดเชื้อต่ำกว่ากลุ่มควบคุมอย่างมีนัยสำคัญ (3.3% vs 23.3%, p = .023) และมีจำนวนวันนอนโรงพยาบาลเฉลี่ยน้อยกว่า (11.70 ± 2.10 vs 14.36 ± 3.16 วัน, p = .001) (3) คะแนนพฤติกรรมส่งเสริมสุขภาพโดยรวมของกลุ่มทดลองสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญ (177.91 ± 7.25 vs 142.39 ± 10.91, p < .001) โดยแตกต่างในหลายมิติ ยกเว้นด้านโภชนาการที่ไม่แตกต่าง (p = .290)
ผลการศึกษาสรุปว่า ระบบการป้องกันการติดเชื้อทางเดินหายใจส่วนล่างแบบบูรณาการในผู้ป่วยมะเร็งศีรษะและคอมีประสิทธิผลในการเพิ่มประสิทธิภาพการเฝ้าระวัง ลดอัตราการติดเชื้อ และลดระยะเวลานอนโรงพยาบาล พร้อมทั้งเสริมพฤติกรรมสุขภาพของผู้ป่วย เหมาะสำหรับประยุกต์ใช้เป็นแนวปฏิบัติในงานพยาบาลเพื่อยกระดับความปลอดภัยของผู้ป่วยอย่างยั่งยืน
