การศึกษาเปรียบเทียบประสิทธิศักย์ของการนวดไทยรีดเส้นด้วยน้ำมันร่วมกับการประคบร้อนสมุนไพรกับการนวดไทยรีดเส้นด้วยน้ำมันอย่างเดียวต่อการลดอาการปวดกล้ามเนื้อ ในผู้ป่วยกลุ่มอาการปวดกล้ามเนื้อและเนื้อเยื่อพังผืด

บทคัดย่อ

บทนำและวัตถุประสงค์: อาการปวดเรื้อรังโดยเฉพาะในผู้ที่มีอาการ myofascial pain syndrome (MPS) หรือกลุ่มอาการปวดกล้ามเนื้อและเนื้อเยื่อพังผืด ซึ่งมีลักษณะเด่นคือการเกิดจุดกดเจ็บ (trigger point: TrP) ภายในกล้ามเนื้อ เป็นปัญหาสุขภาพที่ส่งผลกระทบต่อคุณภาพชีวิตและมีแนวโน้มการกลับมาเป็นซ้ำสูง โดยเฉพาะบริเวณกล้ามเนื้อทราพีเซียส (trapezius) การศึกษานี้มีวัตถุประสงค์เพื่อเปรียบเทียบประสิทธิศักย์ของการนวดไทยรีดเส้นด้วยน้ำมันร่วมกับการประคบร้อนสมุนไพรกับการนวดไทยรีดเส้นด้วยน้ำมันเพียงอย่างเดียว ในการบรรเทาอาการปวดเรื้อรังจาก MPS บริเวณกล้ามเนื้อทราพีเซียส


วิธีการศึกษา: การศึกษานี้เป็นการวิจัยเชิงทดลองแบบสุ่มมีกลุ่มควบคุม (randomized controlled trial; RCT) ในอาสาสมัครที่มีอาการ MPS บริเวณกล้ามเนื้อทราพีเซียส อายุระหว่าง 25–55 ปี จำนวน 60 คน แบ่งเป็น 2 กลุ่ม กลุ่มละ 30 คน ด้วยวิธี stratified randomization โดยกลุ่มทดลองได้รับการนวดไทยรีดเส้นด้วยน้ำมัน 1 ชั่วโมง ด้วยเทคนิครีดเส้นซึ่งเป็นการนวดแบบเชลยศักดิ์ เป็นภูมิปัญญาภาคกลางของไทยโดยนวดเน้นตามเส้นประธานสิบ มีเทคนิคการลงน้ำหนักและรูปแบบการนวดใกล้เคียงกับการนวดสวีดิช (Swedish massage) ที่เป็นสากล ร่วมกับการประคบร้อนสมุนไพรตามสูตรมาตรฐาน 20–30 นาที และกลุ่มควบคุมได้รับการนวดไทยรีดเส้นด้วยน้ำมันเพียงอย่างเดียว 1 ชั่วโมง ทั้งสองกลุ่มได้รับการรักษา 1 ครั้งต่อสัปดาห์ ต่อเนื่อง 3 สัปดาห์ ประเมินผลก่อนและหลังการรักษาด้วย แบบวัดระดับความปวด visual analog scale (VAS), ดัชนีวัดความพร่องความ สามารถของคอ Neck Disability Index (NDI), ระดับขีดกั้นความรู้สึกเจ็บปวดด้วยแรงกด (pressure pain threshold หรือ PPT) บริเวณ trigger point (TrP), การรับรู้การดีขึ้นของอาการปวดของอาสาสมัคร และแบบสอบถามความพึงพอใจ ทั้งนี้ ในระหว่างการศึกษา มีอาสาสมัครกลุ่มทดลองถอนตัว 1 ราย หลังจากนวดครั้งที่ 1 เนื่องจากเจ็บป่วยส่วนบุคคลที่ไม่เกี่ยวข้องกับหัตถการ ผู้วิจัยจึงดำเนินการวิเคราะห์ข้อมูลเฉพาะผู้ที่เข้าร่วมครบตามกำหนด (per–protocol analysis) รวมทั้งสิ้น 59 ราย (กลุ่มทดลอง 29 ราย และกลุ่มควบคุม 30 ราย)


ผลการศึกษา: ข้อมูลพื้นฐานของอาสาสมัครทั้ง 59 ราย ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p > 0.05) หลังการรักษา 3 สัปดาห์ ทั้งสองกลุ่มมีระดับความปวด (VAS) และดัชนี NDI ลดลงอย่างมีนัยสำคัญทางสถิติ (p < 0.05) โดยกลุ่มทดลองมีคะแนน VAS ลดลงเฉลี่ย 4.55 คะแนน และ NDI ลดลงเฉลี่ย 14.17 คะแนน ขณะที่กลุ่มควบคุมมีคะแนน VAS ลดลงเฉลี่ย 4.17 คะแนน และ NDI ลดลงเฉลี่ย 11.40 คะแนน ซึ่งไม่พบความแตกต่างระหว่างกลุ่มอย่างมีนัยสำคัญทางสถิติ สำหรับค่า PPT เพิ่มขึ้นอย่างมีนัยสำคัญในทุกตำแหน่ง (p < 0.001) โดยเฉพาะบริเวณ TrP2 (ส่วนบน) ที่ตอบสนองสูงสุด ซึ่งกลุ่มทดลองมีแนวโน้มการเพิ่มขึ้นของค่า PPT สูงกว่ากลุ่มควบคุมเล็กน้อยแต่ไม่แตกต่างทางสถิติ  ด้านการรับรู้การดีขึ้นของอาการปวด (perceived pain improvement) อาสาสมัครส่วนมากในกลุ่มทดลอง (ร้อยละ 55.20) รับรู้การดีขึ้นระดับปานกลาง ขณะที่กลุ่มควบคุมส่วนมาก (ร้อยละ 63.34) รู้สึกมีอาการดีขึ้นระดับสูง ซึ่งไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติ (p = 0.174 ซึ่งไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p = 0.174) ทั้งนี้ กลุ่มทดลองมีความพึงพอใจระดับมากที่สุดร้อยละ 100.00 และกลุ่มควบคุมร้อยละ 96.70 (p = 1.000)


อภิปรายผล: หัตถบำบัดทั้งสองวิธีสามารถลดระดับความปวด (VAS) และความบกพร่องของคอ (NDI) ได้อย่างมีประสิทธิผลหลังรักษา 3 สัปดาห์ โดยแรงกดเชิงกลจากการนวดลึกช่วยคลายเนื้อเยื่อพังผืดที่หดรั้ง (myofascial release) กระตุ้นการไหลเวียนเลือด และส่งสัญญาณไปยับยั้งกระบวนการรับความเจ็บปวดในระดับไขสันหลังตามทฤษฎีควบคุมประตู (gate control theory) ร่วมกับการกระตุ้นระบบประสาทพาราซิมพาเทติก แม้ผลการวิเคราะห์ทางสถิติระหว่างสองกลุ่มจะไม่แตกต่างกัน (p > 0.05) แต่กลุ่มประคบสมุนไพรร่วมด้วยมีแนวโน้มลดปวดได้มากกว่าเล็กน้อย อาจเนื่องจากผลของความร้อนที่ช่วยลดความหนืดของเนื้อเยื่อ เกี่ยวพัน (connective tissue viscosity) ร่วมกับฤทธิ์ทางเภสัชวิทยาต้านการอักเสบของไพลและขมิ้นชัน นอกจากนี้ การเพิ่มขึ้นอย่างมีนัยสำคัญของค่าขีดกั้นความเจ็บปวด (PPT) ทุกตำแหน่ง (p < 0.001) โดยเฉพาะบริเวณกล้ามเนื้อทราพีเซียส ส่วนบน (TrP2) ยืนยันประสิทธิผลของทั้งการนวดไทยรีดเส้นด้วยน้ำมันเพียงอย่างเดียวและการนวดร่วมกับการประคบร้อนสมุนไพร ในการลดความไวต่อความเจ็บปวดของกล้ามเนื้อทราพีเซียส ได้อย่างชัดเจน


ข้อสรุปและข้อเสนอแนะ: ทั้งการนวดไทยรีดเส้นด้วยน้ำมันร่วมกับการประคบร้อนสมุนไพรและการนวดไทยรีดเส้นด้วยน้ำมันเพียงอย่างเดียว มีประสิทธิผลในการลดความปวดและเพิ่มความสามารถของคอได้อย่างมีนัยสำคัญ แม้ไม่พบความแตกต่างระหว่างกลุ่มในเชิงสถิติ ทั้งสองวิธีเป็นทางเลือกที่ปลอดภัย ราคาไม่แพง และเหมาะสำหรับผู้ที่มีอาการปวดเรื้อรังจากพฤติกรรมใช้งานกล้ามเนื้อซ้ำ ๆ
