การทดสอบการระคายเคืองเฉียบพลันทางผิวหนัง/การกัดกร่อนในกระต่าย ของเจลสารสกัดทองพันชั่งที่มี rhinacanthin 0.2%

บทคัดย่อ

บทนำและวัตถุประสงค์ : ทองพันชั่ง (Rhinacanthus nasutus (L.) Kurz) เป็นไม้พุ่มในวงศ์ Acanthaceae พบมากในเขตร้อนชื้น เช่น เอเชียตะวันออกเฉียงใต้ จีนตอนใต้ และอินเดีย ในทางการแพทย์แผนไทยส่วนใบและรากของทองพันชั่งถูกนำมาใช้รักษาโรคผิวหนัง ได้แก่ ผื่นคัน กลาก และเกลื้อน โดยมีฤทธิ์ต้านเชื้อรากลุ่มเดอร์มาโตไฟต์ได้ดี ส่วนของรากพืชมีสารไรนาแคนทิน–ซี (rhinacanthin C) ซึ่งเป็นสารในกลุ่ม แนปโทควิโนน (napthoquinone) เป็นสารออกฤทธิ์หลักในการต้านเชื้อรา การศึกษาที่ผ่านมามีการพัฒนาเจลสารสกัดรากทองพันชั่งเป็นผลิตภัณฑ์ยาจากสมุนไพรสำหรับใช้ภายนอกเพื่อรักษาโรคผิวหนังจากเชื้อรา โดยตำรับมีปริมาณไรนาแคนทิน–ซี ร้อยละ 0.1 โดยน้ำหนัก และมีข้อมูลยืนยันความปลอดภัยทั้งในสัตว์ทดลองและมนุษย์ อย่างไรก็ตาม ตำรับเจลสารสกัดรากทองพันชั่งที่มีปริมาณสารไรนาแคนทิน–ซี ในความเข้มข้นที่สูงขึ้น เช่น มีไรนาแคนทิน–ซี ร้อยละ 0.2 โดยน้ำหนัก ยังไม่มีข้อมูลด้านความปลอดภัย การศึกษานี้จึงมีวัตถุประสงค์เพื่อทดสอบการระคายเคืองทางผิวหนัง/การกัดกร่อนของเจลสารสกัดรากทองพันชั่งที่มีปริมาณสารไรนาแคนทิน–ซี ร้อยละ 0.2 โดยน้ำหนัก ในกระต่าย เพื่อให้ได้ข้อมูลด้านความปลอดภัยของตำรับเบื้องต้นก่อนนำไปศึกษา dose response ในผู้ป่วย


วิธีการศึกษา : สกัดรากแห้งของทองพันชั่งและหาปริมาณสารไรนาแคนทิน–ซี ด้วยวิธี RP–HPLC เตรียมเจลสารสกัดรากทองพันชั่งที่มีปริมาณสารไรนาแคนทิน–ซี ร้อยละ 0.2 โดยน้ำหนัก และประเมินคุณภาพของตำรับ ได้แก่ การตรวจลักษณะทางกายภาพ ค่า pH การวิเคราะห์ปริมาณสารสำคัญในตำรับ รวมถึงตรวจสอบการปนเปื้อนเชื้อจุลินทรีย์และโลหะหนัก จากนั้นนำเจลสารสกัดรากทองพันชั่งไปทดสอบการระคายเคืองทางผิวหนัง/การกัดกร่อนด้วยวิธีทดสอบตาม OECD Guideline No. 404 ในกระต่ายสายพันธุ์ New Zealand White โดยเริ่มจากการทดสอบการกัดกร่อนผิวหนัง (skin corrosion test) ในกระต่าย 1 ตัว และติดตามอาการทางผิวหนัง เช่น รอยแดง (erythema) และบวม (oedema) หรือสัญญาณของผิวหนังถูกทำลาย และอาการไม่พึงประสงค์อื่น หากไม่พบการกัดกร่อน จึงดำเนินการทดสอบการระคายเคืองผิวหนัง (skin irritation test) ในกระต่ายอีก 2 ตัว เพื่อประเมินการก่อให้เกิดการระคายเคือง


ผลการศึกษา : เจลสารสกัดรากทองพันชั่ง ร้อยละ 0.2 (0.2% rhinacanthin C) ที่เตรียมได้มีลักษณะทางกายภาพเหมาะสม ปริมาณสารสำคัญไรนาแคนทิน–ซี อยู่ในช่วงที่กำหนด 90.0 – 110.0% (90.0 – 110.0% label amount) ไม่พบการปนเปื้อนเชื้อจุลินทรีย์ หรือโลหะหนักเกินค่ามาตรฐาน และมีค่า pH 5.5 ซึ่งเหมาะสมต่อการใช้กับผิวหนัง ผลการทดสอบการกัดกร่อนผิวหนังไม่พบการเปลี่ยนแปลงของสภาพผิวหนัง แสดงว่าตำรับไม่ก่อให้เกิดการกัดกร่อน การทดสอบการระคายเคืองผิวหนังพบว่าตำรับมีค่าเฉลี่ยของการแดงในสัตว์แต่ละตัวอยู่ในช่วง 0.67 – 1.00 และมีค่าเฉลี่ยของการบวมอยู่ในช่วง 0–0.67 ซึ่งอยู่ในช่วง 0 ถึง <1.5 ตามเกณฑ์การประเมิน แสดงให้เห็นว่าเจลสารสกัดรากทองพันชั่งที่มีไรนาแคนทิน–ซี ร้อยละ 0.2 โดยน้ำหนัก ไม่ก่อการระคายเคืองทางผิวหนัง


อภิปรายผล : แม้ว่าผลการทดสอบจะไม่พบการระคายเคืองทางผิวหนังหรือกัดกร่อน แต่ค่าเฉลี่ยของการแดงและการบวมที่พบในสัตว์ทดลองมีค่าสูงกว่าที่เคยรายงานในตำรับเจลสารสกัดรากทองพันชั่งที่มีไรนาแคนทิน–ซี ร้อยละ 0.1 โดยน้ำหนัก ซึ่งมีค่าเฉลี่ยของการแดงอยู่ที่ 0.33 – 0.67 และไม่พบการบวม ผลการศึกษานี้ชี้ให้เห็นว่าการเพิ่มความเข้มข้นของสารไรนาแคนทิน–ซี อาจมีผลต่อระดับการตอบสนองของผิวหนัง ดังนั้นตำรับเจลสารสกัดรากทองพันชั่งที่มีปริมาณสารไรนาแคนทิน–ซี สูงกว่าร้อยละ 0.2 โดยน้ำหนัก อาจมีความเสี่ยงต่อการก่อให้เกิดการระคายเคืองที่ผิวหนังเพิ่มขึ้น


ข้อสรุป และข้อเสนอแนะ : เจลสารสกัดรากทองพันชั่งที่มีปริมาณสารไรนาแคนทิน–ซี ร้อยละ 0.2 โดยน้ำหนัก ไม่ก่อการระคายเคืองทางผิวหนังและไม่ก่อให้เกิดการกัดกร่อนในกระต่ายสายพันธุ์ New Zealand White ในการทดสอบด้วยวิธีตาม OECD Guideline No. 404 การค้นพบนี้ชี้ให้เห็นว่าตำรับดังกล่าวมีความปลอดภัยเบื้องต้นสำหรับการใช้ทาผิวหนัง และสามารถนำไปใช้เป็นข้อมูลสนับสนุนการศึกษาในระดับคลินิกเพื่อประเมินประสิทธิผลและความปลอดภัยของตำรับในการรักษาโรคผิวหนังจากเชื้อราในผู้ป่วยได้ต่อไป
