การรักษาภาวะไหล่ติดด้วยวิธีฝังเข็มอุ่น

บทคัดย่อ
บทนำและวัตถุประสงค์: ภาวะไหล่ติด (Frozen shoulder หรือ Adhesive capsulitis of shoulder) เป็นปัญหาที่พบได้บ่อยในเวชปฏิบัติ มีอาการปวดข้อไหล่เรื้อรัง การเคลื่อนไหวของข้อไหล่ลดลง ในทางการแพทย์แผนจีนเรียกว่าโรคปี้เจิ้ง (Bi Zheng) ปัจจุบันศาสตร์การแพทย์แผนจีนมีการศึกษาประสิทธิผลทางคลินิกในการรักษาภาวะไหล่ติดด้วยวิธีฝังเข็มอุ่น การศึกษานี้มีวัตถุประสงค์ เพื่อศึกษาข้อมูลประสิทธิผลและทบทวนแนวทางการรักษาภาวะไหล่ติดด้วยการฝังเข็มอุ่น
วิธีการศึกษา: การศึกษาในครั้งนี้เป็นการวิจัยเอกสาร (documentary research) โดยทบทวนวรรณกรรม
(literature review) จากฐานข้อมูลงานวิจัย online database CNKI (China National Knowledge Infrastructure) ของ
สาธารณรัฐประชาชนจีนที่มีการเผยแพร่ไว้ระหว่าง มกราคม 2555 จนถึง ธันวาคม 2565 คำสำคัญที่ใช้ คือ “ฝังเข็มอุ่น” และ “ภาวะไหล่ติด” โดยได้คัดกรองเฉพาะบทความที่เป็นการทดลองแบบสุ่มและมีกลุ่มควบคุมเกี่ยวกับประสิทธิผลของการรักษาภาวะไหล่ติดด้วยวิธีการฝังเข็มอุ่นเป็นหลัก
ผลการศึกษา: การฝังเข็มอุ่นมีประสิทธิผลในการลดอาการปวด ลดอักเสบ และฟื้นฟูการเคลื่อนไหวข้อไหล่ในผู้ป่วยภาวะไหล่ติด โดยมีการนำเทคนิคและวิธีการฝังเข็มที่หลากหลายรูปแบบมาใช้ศึกษาเปรียบเทียบ ได้แก่ การฝังเข็มแบบเจียนซานเจิน (เข็มไหล่สามเล่ม) และการฝังเข็มแบบ Simultaneous needling
อภิปรายผล: จากการทบทวนวรรณกรรม พบว่า การเลือกใช้จุดฝังเข็มในภาวะไหล่ติดมุ่งเน้นไปที่จุดฝังเข็มบริเวณข้อไหล่และแขนเป็นหลัก ได้แก่จุด Jianyu (LI15), Quchi (LI11) และ Hegu (LI4) ที่อยู่บนเส้นลมปราณมือหยางหมิงลำไส้ใหญ่ จุด Jianzhen (SI9) ที่อยู่บนเส้นลมปราณมือไท่หยางลำไส้เล็ก Jianliao (TE14) และ Waiguan (TE5) ที่อยู่บนเส้นลมปราณมือเส้าหยางซานเจียว จุด Jianqian (EX-UE12) ที่เป็นจุดพิเศษบนแขน และจุดกดเจ็บ ประสิทธิผลการรักษาโดยรวมและคะแนนแบบประเมิน Visual Analogue Scale (VAS), The Constant-Murley Score, Barthel index, Pain Rating Index (PRI), Melle score of Evaluation of Shoulder Joint และ Function Japanese Orthopedic Association Scores ของกลุ่มฝังเข็มอุ่นดีกว่ากลุ่มเปรียบเทียบ อย่างมีนัยสำคัญทางสถิติ P < 0.05
ข้อสรุปและข้อเสนอแนะ: การฝังเข็มอุ่นมีประสิทธิผลในการรักษาภาวะไหล่ติด สามารถนำไปเป็นแนวทางในการรักษาผู้ป่วยทางคลินิก
