บทคัดย่อ

โครงการนี้มีวัตถุประสงค์เพื่อพัฒนาและศึกษาผลของการใช้รูปแบบการพยาบาลผู้ติดฝิ่นบนพื้นที่สูง กระบวนการพัฒนาใช้แนวคิดของสติงเกอร์ (Stringer) มี 3 ขั้นตอน คือ ค้นหาปัญหา (look) คิดพิจารณา (think) และลงมือปฏิบัติ (act) ดำเนินการในปีงบประมาณ 2559-2562 ในพื้นที่ 6 อำเภอ 2 จังหวัด คือ อำเภอเชียงดาว ไชยปราการ แม่แตง อมก๋อย และเวียงแหง (จ.เชียงใหม่) และอำเภอแม่ระมาด (จ.ตาก)


กลุ่มเป้าหมายมี 3 กลุ่ม ได้แก่



  1. พยาบาลและบุคลากรสาธารณสุข จำนวน 21 คน




  2. บุคลากรเครือข่าย จำนวน 49 คน




  3. ผู้ติดฝิ่น จำนวน 565 คน คัดเลือกแบบเฉพาะเจาะจง



เครื่องมือที่ใช้เก็บรวบรวมข้อมูล ได้แก่ แบบวัดความรู้ก่อน-หลังการอบรม แบบสอบถามคุณภาพชีวิต (มีค่าความเชื่อมั่นเท่ากับ 0.83) และแบบบันทึกข้อมูลผลการดำเนินงาน วิเคราะห์ข้อมูลด้วย ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติทดสอบค่าที


ผลการศึกษา พบว่า:



  1. รูปแบบการพยาบาลผู้ติดฝิ่นบนพื้นที่สูง ประกอบด้วย:



    • แนวทางการมีส่วนร่วมของภาคีเครือข่าย




    • การพยาบาลผู้ติดฝิ่น





  2. ผลของการใช้รูปแบบการพยาบาลฯ:



    • ความรู้เกี่ยวกับการลดอันตรายจากการใช้ยาเสพติด และการจัดตั้งศูนย์ลดอันตรายฯ ของพยาบาล บุคลากรสาธารณสุข และบุคลากรเครือข่ายภายหลังการอบรมสูงกว่าก่อนการอบรมอย่างมีนัยสำคัญทางสถิติที่ระดับ .05




    • มีการจัดตั้งศูนย์ลดอันตรายจากการใช้ยาเสพติด (drop-in center) เพิ่มขึ้นจาก 14 แห่งเป็น 27 แห่ง




    • ผู้ติดฝิ่นเข้ารับการบำบัด รวมทั้งสิ้นจำนวน 2,984 คน และคงอยู่ในระบบการบำบัดร้อยละ 80.03




    • เลิกเสพฝิ่นและเมทาโดน ร้อยละ 12.30




    • มีคุณภาพชีวิตโดยรวมอยู่ในระดับมาก
