การพัฒนาโปรแกรมการบำบัดแบบบูรณาการสำหรับผู้ป่วยโรคติดสุราที่มีความเสี่ยง ฆ่าตัวตาย

บทคัดย่อ
การวิจัยและพัฒนานี้ มีวัตถุประสงค์ เพื่อศึกษาสถานการณ์ พัฒนาและประเมินประสิทธิผลโปรแกรมการบำบัดแบบบูรณาการสำหรับผู้ป่วยโรคติดสุราที่เสี่ยงฆ่าตัวตาย ดำเนินการ 3 ระยะ ได้แก่ (1) ศึกษาสถานการณ์ (2) พัฒนาโปรแกรมฯ และ (3) ประเมินประสิทธิผลของโปรแกรม กลุ่มตัวอย่างระยะที่ 1 คือ ผู้ป่วยโรคติดสุรา 10 คน ผู้ดูแล 10 คน และพยาบาลวิชาชีพ 7 คน กลุ่มตัวอย่างระยะที่ 3 คือ ผู้ป่วยโรคติดสุราที่เสี่ยงฆ่าตัวตาย 66 คน สุ่มเข้ากลุ่มทดลอง 33 คน และกลุ่มควบคุม 33 คน เครื่องมือเก็บข้อมูล ได้แก่ (1) แบบสอบถามข้อมูลส่วนบุคคล (2) แบบประเมินปัญหาการดื่มสุรา มีค่าความเชื่อมั่นเท่ากับ 0.86 (3) แบบประเมินการฆ่าตัวตาย 8Q มีค่าความเชื่อมั่นเท่ากับ 0.90 เครื่องมือการวิจัย ได้แก่ โปรแกรมการบำบัดแบบบูรณาการมีค่าความตรงด้านเนื้อหาเท่ากับ 0.93 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สถิติ Wilcoxon signed-rank test และ Mann-Whitney U
ผลการศึกษา พบว่า ผู้ป่วยโรคติดสุราที่เสี่ยงฆ่าตัวตายมีมุมมองทางลบต่อปัญหา ใช้วิธีการแก้ไขปัญหาแบบหลีกหนี หรือหุนหันพลันแล่น และไม่มีแรงจูงใจในการเลิกดื่มสุรา โปรแกรมการบำบัดแบบบูรณาการ เป็นการบำบัดรายบุคคลจำนวน 8 ครั้ง ได้แก่ (1) ปรับเปลี่ยนแรงจูงใจ (2) ส่งเสริมศักยภาพการดูแลตนเองเพื่อป้องกันการติดสุราซ้ำ (3) สร้างความเข้าใจเกี่ยวกับการดื่มสุราและฆ่าตัวตาย (4) ให้ความรู้รูปแบบการบำบัดโดยการแก้ไขปัญหา (5) สร้างความเข้าใจเกี่ยวกับการแก้ปัญหาและวิธีการแก้ปัญหา และ (6-8) ฝึกปรับเปลี่ยนมุมมองและวิธีการจัดการปัญหา การทดสอบประสิทธิผลของโปรแกรมฯ พบว่า ค่าเฉลี่ยคะแนนพฤติกรรมการดื่มและความเสี่ยงฆ่าตัวตายของกลุมทดลองหลังสิ้นสุดโปรแกรมน้อยกว่าก่อนได้รับโปรแกรมอย่างมีนัยสำคัญทางสถิติ (Z -5.01, และ Z -5.02, p < .01 ตามลำดับ) และค่าเฉลี่ยคะแนนพฤติกรรมการดื่มและความเสี่ยงฆ่าตัวตายหลังสิ้นสุดโปรแกรมของกลุ่มทดลองน้อยกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (Z -6.87, และ Z -4.20, p < .01 ตามลำดับ)
